Psychotropic medications and premature ejaculation?
Hello doctor, I am a patient with depression and have been taking LUVOX and Ativan for almost 4 years.
Recently, I noticed that I have been experiencing premature ejaculation during intercourse with my girlfriend.
I am wondering if this could be influenced by these two medications.
Thank you for your response.

Dr. Zhuo Wenhui reply Pharmacology
The relationship between these two medications and premature ejaculation is not strongly supported in the literature.
Premature ejaculation is associated with many factors, and if there are significant concerns, it is advisable to consult a urologist for an evaluation to identify the underlying causes.

Antidepressants, particularly selective serotonin reuptake inhibitors (SSRIs) like Luvox (fluvoxamine), are known to have various sexual side effects, including premature ejaculation (PE). While these medications are effective in treating depression and anxiety disorders, they can also impact sexual function in both men and women.
In your case, the use of Luvox may be contributing to your experience of premature ejaculation. SSRIs are known to increase serotonin levels in the brain, which can lead to delayed ejaculation in some individuals. However, in certain cases, this alteration in serotonin levels can also result in sexual dysfunction, including PE. The exact mechanism is not fully understood, but it is believed that the modulation of serotonin affects the pathways involved in sexual arousal and ejaculation.
Ativan (lorazepam), a benzodiazepine used primarily for anxiety, can also influence sexual function, although its effects are generally less pronounced compared to SSRIs. Benzodiazepines can lead to sedation and decreased libido, which might indirectly affect sexual performance, but they are not typically associated with premature ejaculation.
It's important to note that sexual side effects can vary significantly from person to person. Some individuals may experience increased sexual dysfunction when taking SSRIs, while others may not notice any changes. Additionally, psychological factors such as anxiety about sexual performance can also contribute to premature ejaculation, creating a complex interplay between medication effects and psychological states.
If you suspect that your medications are affecting your sexual function, it is crucial to discuss this with your healthcare provider. They may consider adjusting your dosage, switching to a different antidepressant with a lower risk of sexual side effects, or adding another medication to help manage the premature ejaculation. Options such as certain antidepressants that are less likely to cause sexual dysfunction, like bupropion, or medications specifically designed to treat PE, may be explored.
In the meantime, practicing relaxation techniques, improving communication with your partner, and focusing on foreplay may help alleviate some of the anxiety surrounding sexual performance. Behavioral techniques, such as the "stop-start" method or the "squeeze" technique, can also be beneficial in managing premature ejaculation.
In summary, while Luvox and Ativan can potentially contribute to premature ejaculation, it is essential to have an open dialogue with your healthcare provider to explore your symptoms and consider appropriate adjustments to your treatment plan. Addressing both the pharmacological and psychological aspects of your situation will be key to improving your sexual health and overall well-being.
