Penile fungal infection?
Dear Dr.
Hsu,
I have had persistent sensitivity in the area of the glans near the coronal sulcus for a long time.
Specifically, when I am not fully erect, I experience pain during masturbation, and there is a white, thin film-like substance in the coronal sulcus.
When I rub it gently with my fingers, white flakes come off, and sometimes there is an unpleasant odor.
I wash daily, but the same white flakes still appear every day.
I used to wash my penis with soap or body wash, but after learning that this might weaken the skin, I have switched to using only water.
I even tried applying some baby oil after showering, but the white flakes still appear.
I would like to ask the doctor:
1.
Is it possible that I have a fungal infection? If so, what should I do?
2.
Could the previous use of soap have caused the skin of the penis to weaken? If so, how can I restore the skin to a normal condition?
Thank you very much, Dr.
Hsu, for taking the time to respond.
I greatly appreciate it.

Y Forest: Hello, the flakes on the penis are similar to dandruff; they are produced daily.
When excessive, they can form a white film covering the glans and the foreskin.
Of course, sometimes a fungal infection can present similar symptoms, but it is usually accompanied by more itching, and upon retraction of the foreskin, the glans may appear red and painful.
This could serve as a basis for diagnosis.

More Info
Hello Arsen,
Thank you for your detailed inquiry regarding your concerns about penile sensitivity and the potential for fungal infections. Based on your description, it sounds like you may be experiencing symptoms related to a fungal infection, specifically candidiasis, which is commonly caused by the yeast Candida. Let’s address your questions one by one.
1. Could this be a fungal infection?
Yes, the symptoms you describe—sensitivity in the glans (head of the penis), the presence of a white film, and the production of white flakes—are consistent with a fungal infection. Candidiasis can cause irritation, itching, and a characteristic white discharge that resembles cottage cheese. The odor you mentioned could also be associated with a fungal infection. It’s important to note that fungal infections thrive in warm, moist environments, and the genital area can be particularly susceptible if not properly cared for.
To address this potential infection, I recommend consulting a healthcare provider for an accurate diagnosis. They may perform a physical examination and possibly take a sample of the discharge for laboratory analysis. If a fungal infection is confirmed, antifungal treatments, such as topical creams or oral medications, can be effective in clearing the infection.
2. Could previous use of soap or body wash have caused skin sensitivity?
Yes, using harsh soaps or body washes can lead to irritation and sensitivity of the delicate skin on the penis. Many commercial soaps contain fragrances and other chemicals that can disrupt the natural balance of bacteria and yeast in the genital area, leading to dryness or irritation. This could explain the increased sensitivity you are experiencing.
To help restore the health of your penile skin, consider the following steps:
- Gentle cleansing: Stick to using warm water for cleaning the area. If you feel the need to use a cleanser, choose a mild, fragrance-free soap specifically designed for sensitive skin.
- Moisturizing: After bathing, consider applying a gentle, hypoallergenic moisturizer to the area to help maintain skin hydration. Avoid products with fragrances or alcohol, as these can further irritate the skin.
- Avoid irritants: Refrain from using baby oil or any other oil-based products that may trap moisture and create an environment conducive to fungal growth. Instead, allow the area to dry completely after washing.
- Loose-fitting underwear: Wearing breathable, loose-fitting underwear can help reduce moisture buildup and irritation.
In addition to these measures, it’s crucial to maintain good hygiene practices, such as regular washing and ensuring the area is dry after bathing. If symptoms persist despite these changes, or if you notice any worsening of your condition, please seek medical advice promptly.
In conclusion, while it is possible that you are experiencing a fungal infection, only a healthcare professional can provide a definitive diagnosis and appropriate treatment. It’s important to address these symptoms early to prevent any complications and to ensure your comfort and health.
Best wishes for your health, and I hope you find relief soon!
Sincerely,
