Frenulum of the prepuce
Hello, Doctor Hsu.
I have some questions I would like to ask you.
Yesterday, while I was taking a shower, I pulled back my foreskin and noticed some small white spots inside.
These white spots seem to be located only under the foreskin, and I can only see them when I retract it; the rest of the area looks normal.
Could you give me an idea of what this might be? There is no itching or pain, just these small white spots.
I didn't shower the day before, so could it be related to sweat? I've never experienced this before, so I'm quite worried.
Please help me with your answer as soon as possible.
Thank you.

Finally: Based on your description, it seems to be smegma, which is a normal occurrence.
Generally, we recommend cleaning it daily by retracting the foreskin, but avoid using soap; just water or body wash will suffice.
Sometimes, if smegma has been present for a long time, it may adhere to the skin and require some effort with a towel to remove.
If you are still concerned, it is best to seek medical attention.
PS: I apologize for the delayed response due to being out of the country and attending meetings for a week.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Understanding white bumps under the foreskin can be a source of concern for many individuals, especially when they notice something unusual that they haven't experienced before. Based on your description, it sounds like you are observing small white bumps that are only visible when you retract the foreskin, and they are not associated with any itching or pain.
The white bumps you are seeing could be several things, but one common explanation is the presence of pearls or Fordyce spots. These are small, painless, pale bumps that can appear on the shaft of the penis or under the foreskin. They are sebaceous glands that are not associated with hair follicles and are completely normal. They can appear more prominent in some individuals and may be more noticeable after sweating or during times of increased skin sensitivity.
Another possibility is smegma, which is a combination of dead skin cells, oils, and other bodily secretions that can accumulate under the foreskin if proper hygiene is not maintained. Smegma can appear as a white, cheesy substance and is typically harmless, but it can lead to irritation or infection if not cleaned regularly.
In some cases, white bumps could also be indicative of a condition known as balanitis, which is inflammation of the glans (the head of the penis) and can occur due to various factors, including poor hygiene, infections, or skin conditions. However, since you mentioned that there is no itching or pain, this is less likely.
It is essential to maintain good hygiene by gently cleaning the area under the foreskin with warm water during bathing. If you notice any changes in the appearance of the bumps, such as an increase in size, color change, or if they become painful or itchy, it would be wise to consult a healthcare professional for a thorough examination.
While it is understandable to feel anxious about new symptoms, many conditions related to the genital area are benign and can be easily managed. If you are still concerned or if the bumps persist, I recommend scheduling an appointment with a urologist or a dermatologist who specializes in skin conditions. They can provide a definitive diagnosis and reassurance, as well as discuss any necessary treatment options if required.
In summary, the white bumps you are observing under the foreskin are likely benign, such as Fordyce spots or smegma. Maintaining proper hygiene is crucial, and if any concerning symptoms arise, seeking medical advice is always the best course of action. Remember, it’s always better to be cautious and informed about your health.
