Does taking Deanxit affect urination?

Hello, Doctor.
My father is in his fifties and currently has an issue with benign prostatic hyperplasia (BPH), which he is managing with DOXAZOSIN.
Recently, due to emotional instability, his physician prescribed Deanxit for him.
However, it has become very apparent that after taking the medication, he experiences difficulty urinating and has to wait a while before he can urinate again.
Could you please advise if Deanxit or other antidepressants could affect urination or exacerbate his BPH condition? A worried son.

Xiaoshun de erzi, 30~39 year old female. Ask Date: 2005/11/04

Dr. Wang Chongren reply Psychiatry

Dear respectful son,
Dr.
Wang Chong-Jen from the Tainan Hospital of the Department of Health has noted that some individuals may experience urinary difficulties when taking Deanxit, likely due to its anticholinergic side effects.
It may be advisable to consider switching to a lower-dose, newer generation medication, such as Prozac (Fluoxetine), Aurorix (Moclobemide), or Mesyrel (Trazodone).
In cases of acute urinary retention, emergency intervention such as catheterization should be performed.
The Department of Psychiatry at Tainan Hospital wishes you good health and all the best.

Deanxit, a combination medication primarily used for treating anxiety and depression, can indeed have implications for urination, particularly in patients with prostate enlargement, also known as benign prostatic hyperplasia (BPH). Your father's experience of difficulty urinating after taking Deanxit is not uncommon, as certain medications in this class can exacerbate urinary retention issues.

The primary reason for this side effect is the anticholinergic properties of some components in Deanxit. Anticholinergic medications can inhibit the action of acetylcholine, a neurotransmitter that plays a crucial role in bladder function. This inhibition can lead to relaxation of the bladder muscle, making it more difficult to initiate urination. In patients with pre-existing conditions like BPH, where the prostate gland is enlarged and can already obstruct urine flow, the effects of anticholinergic medications can be particularly pronounced, leading to increased urinary retention.

In addition to Deanxit, other antidepressants and anxiolytics can also have similar effects. For instance, medications like tricyclic antidepressants and certain SSRIs (Selective Serotonin Reuptake Inhibitors) may also contribute to urinary difficulties. Therefore, it is essential to consider the overall medication regimen and how each drug may interact with the symptoms of BPH.

If your father is experiencing significant urinary retention, it is crucial to consult with his healthcare provider. They may consider adjusting his medication or switching to alternatives with a lower risk of urinary side effects. For example, newer antidepressants, such as certain SSRIs or SNRIs (Serotonin-Norepinephrine Reuptake Inhibitors), may have a more favorable side effect profile concerning urinary function.

In cases of acute urinary retention, where your father cannot urinate at all, immediate medical intervention may be necessary. This could involve catheterization to relieve the bladder and prevent complications such as bladder distension or urinary tract infections.

Moreover, it is important to monitor any changes in urinary habits closely. If your father experiences increased frequency, urgency, or pain during urination, these symptoms should be reported to his doctor promptly. Additionally, lifestyle modifications, such as fluid management and pelvic floor exercises, may also help alleviate some urinary symptoms associated with BPH.

In summary, Deanxit can affect urination, especially in patients with prostate enlargement. The anticholinergic effects of the medication can lead to urinary retention, exacerbating existing conditions like BPH. It is essential to communicate these side effects to a healthcare provider, who can assess the situation and make necessary adjustments to the treatment plan. Your father's health and comfort are paramount, and addressing these concerns with a professional will help ensure he receives the best possible care.
