Acne ~ Blackheads
Sometimes I get acne that leaves dark spots, resembling freckles, without squeezing them.
A friend mentioned that it might be because my skin is thin.
Is that true? How can I lighten these spots? Sometimes I have bumps that look like acne but feel painful to the touch and disappear the next day.
Why does this happen? Also, how can I reduce blackheads?
Xiao Hai, 10~19 year old female. Ask Date: 2005/11/05
Dr. Zheng Lizhen reply Dermatology
Hello: 1.
After inflammation, there is a possibility of pigment deposition, which is not related to skin thickness.
You can apply some L-ascorbic acid (Vitamin C) and sunscreen to help lighten the pigmentation.
2.
Please visit a dermatology specialty clinic for an accurate diagnosis and treatment.
3.
For blackheads, you can use some alpha hydroxy acids (AHAs) or retinoids to improve the condition.
Sincerely, Dr.

More Info
Acne, blackheads, and skin sensitivity are common skin concerns that can be influenced by various factors, including skin type, environmental conditions, and lifestyle choices. Understanding the underlying causes and effective treatments can help manage these issues effectively.
Causes of Acne and Blackheads
Acne is primarily caused by the clogging of hair follicles with oil (sebum), dead skin cells, and bacteria. When these follicles become blocked, they can lead to the formation of pimples, blackheads, and cysts. Blackheads, specifically, are open comedones that appear black due to oxidation of the sebum when it comes into contact with air. Factors that contribute to acne include:
1. Hormonal Changes: Fluctuations in hormones, particularly during puberty, menstruation, or pregnancy, can increase oil production in the skin.
2. Diet: Some studies suggest that high-glycemic foods and dairy products may exacerbate acne.
3. Stress: Stress can lead to hormonal changes that may worsen acne.
4. Skin Type: Individuals with oily skin are more prone to acne and blackheads.
5. Skin Sensitivity: Thinner skin can be more reactive to environmental factors, leading to increased sensitivity and potential for irritation.
Skin Sensitivity and Reactions
The phenomenon you described, where you experience painful spots that resemble mosquito bites, could be attributed to several factors:
- Allergic Reactions: Your skin may react to certain allergens or irritants, leading to localized inflammation.
- Folliculitis: This is an inflammation of the hair follicles, which can cause red, painful bumps that may resolve quickly.
- Contact Dermatitis: This occurs when the skin comes into contact with an irritant or allergen, leading to redness and discomfort.
Treatment Options
1. For Acne and Blackheads:
- Topical Retinoids: Products containing retinoids (like tretinoin or adapalene) can help unclog pores and reduce the formation of acne and blackheads.
- Salicylic Acid: This beta hydroxy acid helps exfoliate the skin and clear out clogged pores, making it effective for both acne and blackheads.
- Benzoyl Peroxide: This ingredient has antibacterial properties and can help reduce acne-causing bacteria.
- Regular Exfoliation: Gentle exfoliation can help remove dead skin cells and prevent clogged pores. However, be cautious not to over-exfoliate, as this can irritate the skin.
2. For Skin Sensitivity:
- Moisturizers: Use non-comedogenic moisturizers to maintain skin hydration without clogging pores.
- Sunscreen: Protecting your skin from UV rays is crucial, especially if you have sensitive skin.
- Avoid Irritants: Identify and avoid products that may trigger your skin sensitivity, such as harsh cleansers or fragrances.
3. For Hyperpigmentation:
- Vitamin C Serums: These can help brighten the skin and reduce the appearance of dark spots.
- Chemical Peels: Professional treatments can help exfoliate the skin and reduce pigmentation.
- Hydroquinone: This topical agent can lighten dark spots but should be used under the guidance of a dermatologist.
Prevention Strategies
- Maintain a Consistent Skincare Routine: Cleanse your skin daily and use products suitable for your skin type.
- Healthy Diet: Incorporate a balanced diet rich in fruits, vegetables, and whole grains while limiting processed foods.
- Stay Hydrated: Drinking plenty of water can help maintain skin health.
- Manage Stress: Engage in stress-reducing activities such as exercise, meditation, or hobbies.
In conclusion, understanding the causes of acne, blackheads, and skin sensitivity is crucial for effective management. If you continue to experience persistent issues, consulting a dermatologist can provide personalized treatment options tailored to your skin's needs.
