Is it possible that this is a pregnancy?

Hello Doctor: My menstrual cycle in October started on the 14th, and in November it started on the 16th.
My periods are longer than others, about 30 to 40 days...
but sometimes I don’t get my period for a whole month...
to be honest, it’s quite irregular.
Here’s the situation: I had sexual intercourse with my boyfriend on December 11 and 18, both times with withdrawal method.
I originally expected my period to come on the 18th, but it didn’t.
On the 20th, when I went to the bathroom in the morning, I noticed a spot of red blood on my underwear.
I thought my period had started, so I changed to a sanitary pad, but by the afternoon, I saw that the blood was brown and very light in amount.
Over the next few days, I continued to have bleeding, often accompanied by normal red blood.

I remember that in the months following my first period, it was similar; the blood was brown and very light, and it took a few months to return to normal.
Even as I grew older, I still experienced this.
On December 25, I visited a gynecologist, and they performed a urine test, which showed I was not pregnant.
However, the doctor mentioned that the test might not be accurate at this stage and prescribed a medication called "PROVERA," which the doctor said could help stop the bleeding and also induce menstruation.
Indeed, after taking it, the bleeding stopped immediately.
The doctor advised me to wait a week and then retest.
Recently, I have not experienced any early pregnancy symptoms, such as dizziness, nausea, or vomiting.
I would like to ask the doctor if this situation is due to an underlying issue with my body or if there is still a possibility that I could be pregnant.
Thank you!

Xiao Xiao, 10~19 year old female. Ask Date: 2005/12/29

Women with irregular menstrual periods are prone to abnormal bleeding during menstruation.
If a urine pregnancy test has been conducted and the result is negative, "PROVERA," a progestin medication, can be used for the treatment of abnormal menstrual bleeding and to induce menstruation.
After taking "PROVERA," if menstruation does not occur within 1 to 2 weeks, a repeat urine pregnancy test can be performed.
Symptoms such as dizziness, nausea, and vomiting, which may resemble early pregnancy symptoms, do not necessarily indicate pregnancy.

When it comes to understanding whether you could be pregnant, especially with irregular periods, it's essential to consider several factors. Based on your description, your menstrual cycle has been inconsistent, ranging from 30 to 40 days, and sometimes even skipping a month. This irregularity can complicate the assessment of potential pregnancy.

You mentioned having sexual intercourse with your boyfriend on December 11 and 18, both times using the withdrawal method (also known as "pulling out"). While this method can reduce the risk of pregnancy, it is not foolproof. Pre-ejaculate fluid can contain sperm, and if ejaculation occurs near the vaginal opening, there is still a risk of pregnancy.

You anticipated your period around December 18, but it did not arrive as expected. Instead, on December 20, you noticed some bleeding, which you initially thought was your period. However, the blood was brownish and minimal, which can sometimes indicate old blood or spotting rather than a full menstrual flow. Spotting can occur for various reasons, including hormonal fluctuations, implantation bleeding (if pregnancy has occurred), or other underlying health issues.

You visited a gynecologist on December 25, where a urine pregnancy test returned negative. It's important to note that urine tests can sometimes yield false negatives, especially if taken too early. The doctor prescribed Provera, a medication that can help regulate your menstrual cycle and stop abnormal bleeding. This suggests that your doctor is considering hormonal imbalances as a potential cause of your symptoms.

Regarding your question about whether you could still be pregnant despite the negative test and lack of typical early pregnancy symptoms (like nausea or dizziness), it is indeed possible. Some women experience very mild symptoms or none at all in the early stages of pregnancy. However, the absence of symptoms does not rule out pregnancy, especially if your menstrual cycle is irregular.

Given your history of irregular periods and the recent bleeding, it is crucial to follow your doctor's advice. After completing the course of Provera, you should retest for pregnancy as recommended. If your period does not resume or if you continue to experience unusual symptoms, further evaluation may be necessary. This could include blood tests, which are more sensitive than urine tests, or imaging studies to assess your reproductive health.

In conclusion, while the irregularity of your menstrual cycle and the negative pregnancy test suggest that pregnancy is less likely, it is not entirely ruled out. Monitoring your symptoms and following up with your healthcare provider is essential for determining the next steps. If you have any concerns about your menstrual health or potential pregnancy, do not hesitate to reach out to your healthcare provider for personalized advice and support.
