There is a feeling of heaviness and bloating in the right upper abdomen?
Hello, doctor.
I have been experiencing bloating in my left abdomen for quite some time.
I have taken medications from several clinics, but there hasn't been a significant improvement.
Recently, I often feel a sense of heaviness and bloating in my right upper abdomen, and sometimes there is a slight stabbing pain.
I am really worried that there might be an issue with my liver.
I had a health check-up last October, and my liver function was normal, and I do not have hepatitis B.
What could this condition possibly be? Thank you.

Dr. Chen Zhiping reply Gastroenterology and Hepatology
Hello: Liver function is normal, and although there is no hepatitis B, liver and gallbladder issues cannot be completely ruled out.
It is recommended that you undergo an abdominal ultrasound to exclude problems with the liver, gallbladder, and pancreas.
There are many causes of abdominal distension; if it is not accompanied by weight loss, anemia, fever, and if the symptoms fluctuate, it is mostly functional gastrointestinal disorders.
Sincerely, Dr.

More Info
Right upper abdominal discomfort can be concerning, especially when accompanied by symptoms such as bloating, pain, or a feeling of pressure. Based on the information provided, there are several potential causes for your symptoms, and understanding these can help guide you toward appropriate management and treatment.
1. Anatomical Considerations: The right upper quadrant (RUQ) of the abdomen houses several vital organs, including the liver, gallbladder, right kidney, and parts of the intestines. Given that you have experienced discomfort in this area, it is essential to consider the specific organs involved.
2. Possible Causes of RUQ Discomfort:
- Liver Issues: Although you mentioned that your liver function tests were normal during your last check-up, conditions such as fatty liver disease, hepatitis, or liver tumors can still occur without immediate changes in liver enzymes. If you have a history of alcohol use or other risk factors, it may warrant further investigation.
- Gallbladder Problems: Gallstones or cholecystitis (inflammation of the gallbladder) can cause RUQ pain, especially after eating fatty meals. Symptoms may include sharp pain, nausea, and sometimes jaundice if a stone obstructs the bile duct.
- Gastrointestinal Disorders: Conditions like gastritis, peptic ulcers, or functional dyspepsia can lead to discomfort in the upper abdomen. Symptoms may include bloating, nausea, and pain that can mimic gallbladder or liver issues.
- Kidney Issues: Kidney stones or infections can also present as pain in the RUQ, although they typically radiate to the back or flank area.
- Musculoskeletal Pain: Sometimes, discomfort in the abdominal area can stem from muscle strain or rib-related issues, particularly if the pain is sharp and localized.
3. Symptoms to Monitor: It’s important to keep track of any additional symptoms that may arise, such as:
- Changes in bowel habits (diarrhea, constipation)
- Nausea or vomiting
- Fever or chills
- Unexplained weight loss
- Jaundice (yellowing of the skin or eyes)
4. When to Seek Medical Attention: If your symptoms persist or worsen, it is advisable to consult a healthcare professional. They may recommend imaging studies such as an ultrasound or CT scan to visualize the organs in the RUQ and rule out any serious conditions. Blood tests may also be necessary to check for signs of infection, inflammation, or liver dysfunction.
5. Management Strategies: Depending on the underlying cause, treatment may vary:
- For gallbladder issues, dietary modifications (reducing fat intake) and, in some cases, surgical intervention may be necessary.
- If gastrointestinal issues are suspected, medications to reduce stomach acid or treat infections may be prescribed.
- Lifestyle changes, including diet, hydration, and stress management, can also play a significant role in alleviating symptoms.
In conclusion, while your concerns about liver health are valid, there are multiple potential causes for your right upper abdominal discomfort. It is crucial to work closely with a healthcare provider to determine the underlying issue and develop an appropriate treatment plan. Regular follow-ups and monitoring of your symptoms will also be essential in managing your health effectively.
