Penile epidermal issues?
Dear Dr.
Lai,
I hope this message finds you well.
I would like to seek your advice regarding a concerning issue.
Recently, I have noticed that the skin on my penis has become very thin and fragile, making it prone to cracking and bleeding.
After a few days, the wounds heal, but if I touch it forcefully or experience friction during an erection, it immediately cracks open again.
Could you please help me understand what might be causing this? Is it possible that I have a Candida infection, as my wife was diagnosed with one by her gynecologist? Could I have contracted it as well? I would greatly appreciate your guidance on this matter.
Thank you!

Dr. Lai Ronghao reply Urology
Hello: Infection with Neisseria can cause pinpoint red rashes on the male genitalia, which can be quite itchy.
Based on your description, it does not match this symptom.
Thinning of the epidermis may be related to individual constitution, and it is more likely to occur in individuals with diabetes or autoimmune system issues.
I recommend that you visit a urology clinic for further examination and treatment.
Thank you!

Understanding the condition of thin and fragile penile skin is crucial for both physical comfort and overall health. The symptoms you describe—thinning skin that cracks easily, bleeds, and reopens upon contact—can be concerning and may arise from several underlying causes.
One potential cause of thin and fragile skin in the genital area is a condition known as lichen sclerosus. This is a chronic inflammatory skin condition that can lead to thinning of the skin, resulting in white patches and increased fragility. It is more common in women but can also affect men. The skin may become itchy, painful, and prone to tearing, especially during erections or friction.
Another possibility is a fungal infection, such as a candidiasis (yeast infection), which can cause irritation and inflammation of the skin. If your wife has been diagnosed with a yeast infection, it is indeed possible for you to contract a similar infection, especially if there has been intimate contact. Symptoms of a yeast infection can include redness, itching, and discomfort, which may contribute to the fragility of the skin.
Additionally, contact dermatitis could be a factor. This condition occurs when the skin reacts to irritants or allergens, leading to inflammation and sensitivity. Common irritants include soaps, lotions, or even latex from condoms. If you have recently changed any products used in the genital area, this could be a contributing factor.
Another consideration is the impact of certain medical conditions, such as diabetes or hormonal imbalances, which can affect skin integrity. Diabetes, for instance, can lead to poor circulation and skin changes that make the skin more susceptible to injury.
Given the symptoms you are experiencing, it is essential to seek medical advice from a healthcare professional, preferably a dermatologist or urologist. They can perform a thorough examination, possibly including a skin biopsy or cultures, to determine the exact cause of your symptoms.
In the meantime, here are some general recommendations to help manage your condition:
1. Gentle Hygiene: Use mild, fragrance-free soaps and avoid harsh scrubs or exfoliants in the genital area. Pat the area dry gently after bathing.
2. Moisturization: Applying a gentle, hypoallergenic moisturizer can help maintain skin hydration and prevent cracking. Look for products specifically designed for sensitive skin.
3. Avoid Irritants: Identify and avoid any products that may irritate your skin, including certain lubricants, soaps, or laundry detergents.
4. Wear Loose Clothing: Opt for breathable, loose-fitting underwear and clothing to reduce friction and irritation.
5. Monitor Symptoms: Keep track of any changes in your symptoms, including the appearance of new lesions, increased pain, or any discharge. This information will be valuable for your healthcare provider.
6. Avoid Sexual Activity: Until you have been evaluated by a healthcare professional, it may be wise to avoid sexual activity to prevent further irritation or injury to the area.
In conclusion, while the symptoms you are experiencing can be distressing, they are not uncommon, and there are effective treatments available once a proper diagnosis is made. It is crucial to consult with a healthcare provider to ensure you receive the appropriate care tailored to your specific condition.
