Rectal bleeding during hot water sitz baths?

I have hemorrhoids; everything has been normal recently (I have a bowel movement every day; there is no blood in the stool).
Yesterday afternoon, there was no bleeding during my bowel movement; however, last night during a warm sitz bath, I noticed bleeding from the anus, which turned the water red.
Could you please explain why there was bleeding from the anus during the sitz bath? Is it related to the hemorrhoids? Thank you!

Dr. Lin Kecheng reply Surgery

The typical characteristics of internal hemorrhoids are painless rectal bleeding.
Sometimes, it is noticed when wiping the anal area after a bowel movement or when blood drips into the toilet.
Warm sitz baths can reduce swelling of internal hemorrhoids and alleviate discomfort in the anal area, providing only benefits.
If bleeding occurs during a warm sitz bath, it may coincidentally be due to a hemorrhoid flare-up, and the bleeding is unrelated to the sitz bath.
The causes of hemorrhoid flare-ups include constipation, inadequate fluid intake, and consumption of irritating foods (such as spicy foods, staying up late, alcohol, or foods containing alcohol like sesame oil chicken, ginger duck, and lamb hot pot).
It is recommended to visit a colorectal surgeon for a more accurate diagnosis after examination, as well as appropriate treatment.
Wishing you health and happiness.

More Info

Rectal bleeding during warm sitz baths can indeed be concerning, especially for individuals with a history of hemorrhoids. Hemorrhoids are swollen veins in the lower rectum and anus that can cause discomfort, itching, and bleeding, particularly during bowel movements or when pressure is applied to the area. The warm water from a sitz bath can increase blood flow to the area, which may exacerbate any existing hemorrhoids, leading to bleeding.

When you mention that you had no blood during bowel movements but noticed bleeding during the sitz bath, it suggests that the warm water may have caused the hemorrhoids to become more engorged or irritated. The heat can relax the anal sphincter and dilate blood vessels, potentially leading to increased bleeding if there are any existing fissures or hemorrhoids.
It's also important to consider other potential causes of rectal bleeding. While hemorrhoids are a common cause, other conditions such as anal fissures, rectal prolapse, or even more serious issues like colorectal polyps or cancer can also lead to bleeding. However, given your history of hemorrhoids and the context of the bleeding occurring during a sitz bath, it is likely related to that condition.

To manage this situation, here are some recommendations:
1. Avoid Straining: Ensure that you are not straining during bowel movements, as this can aggravate hemorrhoids. A diet high in fiber can help soften stools and reduce straining.

2. Warm Sitz Baths: Continue using warm sitz baths, as they can provide relief from discomfort and help with healing. However, if you notice increased bleeding, it may be wise to reduce the frequency or duration of the baths.

3. Topical Treatments: Over-the-counter creams or ointments specifically designed for hemorrhoids can help reduce inflammation and discomfort. These may contain hydrocortisone or witch hazel.

4. Hydration and Diet: Stay well-hydrated and maintain a diet rich in fruits, vegetables, and whole grains to promote regular bowel movements and prevent constipation.

5. Consult a Healthcare Provider: If the bleeding persists, worsens, or if you experience significant pain, it is crucial to consult a healthcare provider. They may perform a physical examination and possibly recommend further diagnostic tests to rule out other conditions.

6. Avoid Hot Water: While warm baths can be soothing, excessively hot water can lead to increased blood flow and swelling in the area, potentially worsening hemorrhoids.

In summary, while it is likely that the bleeding you experienced during the sitz bath is related to your hemorrhoids, it is essential to monitor your symptoms closely. If you have any concerns or if the bleeding continues, seeking medical advice is the best course of action to ensure proper diagnosis and treatment.
