Anterior cruciate ligament (ACL) tear
Hello Dr.
Hong: I was in a car accident and after examination and MRI, the doctor informed me that I have torn posterior cruciate ligaments in both of my legs.
I would like to ask if I can request to have the surgery to remove the steel plate and screws in my left leg performed together with the ligament reconstruction surgery? Additionally, can I have the ligament reconstruction for both legs done at the same time, or is it only possible to do one leg at a time? Thank you for your assistance.
Xiao Jie, 20~29 year old female. Ask Date: 2006/08/25
Dr. Hong Ximing reply Orthopedics
Hello Xiao Jie: The simultaneous rupture of both cruciate ligaments in your knees indicates a significant injury.
The use of a steel plate and ligament reconstruction is not mutually exclusive.
You can choose to undergo both procedures at the same time or separately.
It is important to discuss these options in detail with your attending physician before making a decision.
Wishing you a speedy recovery.

When considering the possibility of undergoing both ACL reconstruction and hardware removal surgery in the same procedure, there are several factors to take into account. First and foremost, it is essential to consult with your orthopedic surgeon, as they will have the best understanding of your specific medical condition and the intricacies of your injuries.
1. Can ACL Reconstruction and Hardware Removal Be Done Together?
In general, it is feasible to perform ACL reconstruction and hardware removal in the same surgical session, provided that the surgeon deems it safe and appropriate based on your individual circumstances. The decision will depend on several factors, including the stability of the fracture site where the hardware is located, the condition of the surrounding tissues, and the overall health of your knee joint. If the hardware is not interfering with the ACL reconstruction and if the surgeon believes that performing both procedures simultaneously will not increase the risk of complications, then it may be possible to combine them.
2. Considerations for Combined Surgery:
- Recovery Time: Combining surgeries may reduce the overall recovery time, as you would only need to go through the rehabilitation process once instead of twice.
- Anesthesia Risks: Undergoing multiple procedures at once may increase the duration of anesthesia, which carries its own risks, especially for patients with certain health conditions.
- Surgical Complexity: Performing two procedures simultaneously may increase the complexity of the surgery, which could lead to a longer operation time and potentially higher risks of complications.
3. Can Both Knees Be Reconstructed at the Same Time?
Regarding the possibility of reconstructing both ACLs simultaneously, this is generally less common. While it is technically possible, it is usually not recommended due to the increased risk of complications and the challenges associated with postoperative rehabilitation. Each knee requires focused rehabilitation to regain strength and mobility, and managing recovery for both knees at the same time can be quite challenging. Most surgeons prefer to perform ACL reconstruction on one knee at a time to ensure that each knee receives adequate attention during the recovery process.
4. Postoperative Rehabilitation:
Regardless of whether you have one or both procedures done, rehabilitation will be a critical component of your recovery. After ACL reconstruction, a structured rehabilitation program will typically involve physical therapy aimed at restoring range of motion, strength, and function to the knee. The timeline for recovery can vary significantly based on individual factors, but it often takes several months to return to full activity.
5. Consultation with Your Surgeon:
Ultimately, the best course of action is to have a detailed discussion with your orthopedic surgeon. They can provide you with personalized advice based on your medical history, the specifics of your injuries, and their surgical experience. If you have concerns or preferences regarding the timing of your surgeries, be sure to express these during your consultation.
In conclusion, while it is possible to have ACL reconstruction and hardware removal surgery together, the decision should be made collaboratively with your surgeon, considering all relevant factors. Additionally, reconstructing both ACLs at the same time is generally not advisable due to the complexities involved in recovery. Your surgeon will guide you through the best options tailored to your needs.
