Four months after the fistula surgery, there is still discharge?

Hello doctor, I underwent anal fistula surgery at Chang Gung in August, and until now, there is still some discharge from my anus.
Sometimes my stools are hard, and after bowel movements, I experience a bit of pain.
After a while, I notice discharge and when I wipe, I find a yellow, sticky substance that resembles pus.
I'm not sure how to handle this.
Thank you.

Hello: In response to your question, after anal fistula surgery, as long as it is not a complex fistula, the wound generally heals within one to two months post-surgery.
During the healing process, there may be some pain and discharge; however, if there is still discharge after such a long time post-surgery, it is important to assess whether it is due to poor healing or if the fistula was not completely excised.
If it is due to poor healing, patience is required.
If the fistula was not adequately removed, another surgery may be necessary.

After undergoing anal fistula surgery, it is not uncommon to experience some persistent discharge, especially in the weeks following the procedure. The presence of yellow, thick, and sometimes purulent discharge can be concerning, but it is essential to understand the context of your symptoms and what steps you can take to manage them effectively.

Firstly, it is important to recognize that some degree of discharge is expected after fistula surgery. The surgical site may still be healing, and the body can produce fluid as part of the healing process. However, if the discharge is persistent, especially if it is accompanied by pain, changes in color, or an increase in volume, it may indicate an underlying issue such as infection or incomplete healing.

Here are some steps you can take to address your concerns:
1. Monitor Symptoms: Keep a close eye on the characteristics of the discharge. If it becomes increasingly foul-smelling, changes to a more pronounced yellow or green color, or is accompanied by fever, increased pain, or swelling, these could be signs of infection, and you should seek medical attention promptly.

2. Hygiene Practices: Maintain good hygiene around the surgical site. After bowel movements, gently clean the area with mild soap and water. Avoid using harsh soaps or scrubbing the area too vigorously, as this can irritate the skin and exacerbate discharge.

3. Sitz Baths: Regular sitz baths can help soothe the area and promote healing. Soaking in warm water for about 15-20 minutes several times a day can help reduce discomfort and may assist in the drainage of any residual fluid.

4. Dietary Considerations: To prevent hard stools, which can exacerbate pain during bowel movements, ensure you are consuming a high-fiber diet. Incorporate plenty of fruits, vegetables, and whole grains into your meals, and consider staying well-hydrated. If necessary, consult your healthcare provider about using stool softeners.

5. Follow-Up Appointments: It is crucial to attend all follow-up appointments with your surgeon or healthcare provider. They can assess the healing process and determine if any further intervention is needed. If you have not had a follow-up appointment since your surgery, it may be beneficial to schedule one to discuss your ongoing symptoms.

6. Medication: If you are experiencing significant pain, consult your healthcare provider about appropriate pain management options. They may recommend over-the-counter pain relievers or prescribe medication if necessary.

7. Avoid Self-Medication: While it may be tempting to self-medicate with over-the-counter treatments for diarrhea or other gastrointestinal issues, it is essential to consult your healthcare provider before doing so. They can provide guidance on the safest and most effective options based on your specific situation.

In summary, while some discharge after anal fistula surgery can be normal, persistent or concerning symptoms warrant further evaluation by your healthcare provider. By monitoring your symptoms, maintaining good hygiene, and following up with your medical team, you can help ensure a smoother recovery process. If you have any doubts or if your symptoms worsen, do not hesitate to reach out to your healthcare provider for advice and potential treatment options.
