There is a pressure at the back of my head?

For several months now, I've been experiencing a sudden heaviness in the back of my head that makes me feel a bit dizzy.
It's not very severe, but the heaviness often makes me feel sleepy and I struggle to lift my head.
I also find that I dream easily, and when I wake up, I feel like I haven't slept much.
What could be causing this?

A Fu, 10~19 year old female. Ask Date: 2007/01/06

Dr. Ye Boshou reply Neurology

Hello: There seems to be no specific discomfort, and it is difficult to make a diagnosis.
A physical examination and detailed medical history in an outpatient setting are necessary to identify the cause.

The sensation of heaviness at the back of your head, accompanied by feelings of dizziness and fatigue, can be quite concerning and may stem from various underlying causes. Understanding these symptoms is crucial for determining the appropriate course of action.

Firstly, the heaviness you describe could be indicative of tension-type headaches, which are among the most common forms of headaches. These headaches often arise from muscle tension in the neck and scalp, which can be exacerbated by stress, poor posture, or prolonged periods of sitting, especially in environments that are not ergonomically friendly. The feeling of heaviness can also be associated with fatigue, leading to a desire to sleep more, as your body may be signaling that it needs rest to recover from the tension.

Another potential cause for your symptoms could be related to cervical spine issues. Conditions such as cervical spondylosis or muscle strain in the neck can lead to referred pain or a sensation of heaviness in the head. This is particularly relevant if you have a history of neck injuries or if you spend long hours in positions that strain your neck.

Additionally, your symptoms could be linked to migraines or other types of headaches. Migraines can sometimes present with a feeling of pressure or heaviness in the head, along with dizziness and fatigue. They may also be triggered by various factors, including stress, dietary choices, or hormonal changes.

It's also important to consider the possibility of postural hypotension, where changes in posture can lead to dizziness and a feeling of heaviness in the head. This can occur when standing up too quickly or after sitting for extended periods.

Given that you mentioned feeling like you haven't slept well upon waking, this could also suggest that your sleep quality is being affected. Sleep disorders, such as sleep apnea, can lead to excessive daytime sleepiness and a feeling of heaviness in the head upon waking. If your sleep is frequently interrupted or of poor quality, it could contribute to your overall symptoms.

To address these issues, it is advisable to consult a healthcare professional, preferably a neurologist or a primary care physician. They can perform a thorough evaluation, which may include a physical examination, a review of your medical history, and possibly imaging studies like an MRI or CT scan if deemed necessary. This will help rule out any serious underlying conditions.

In the meantime, consider implementing some lifestyle changes that may alleviate your symptoms. Regular physical activity, particularly exercises that strengthen the neck and improve posture, can be beneficial. Additionally, practicing relaxation techniques such as yoga or meditation can help reduce stress and muscle tension. Ensuring you have a comfortable sleep environment and maintaining a regular sleep schedule can also improve your sleep quality.

In summary, while the heaviness at the back of your head and associated symptoms can be distressing, they are often manageable with appropriate medical guidance and lifestyle adjustments. Seeking professional advice will provide you with a clearer understanding of your condition and the best strategies for relief.
