Penile curvature

Hello doctor, last year I underwent corrective surgery for congenital penile curvature at Taipei Hospital in Xinzhuang.
However, a few days ago, I may have overstressed the sutures in the corpus cavernosum, and it seems like they have ruptured.
Now, the shape appears to be curving down again.
When I achieve an erection to the maximum hardness, there is pain at the site of the sutures on the right side of the corpus cavernosum.
What should I do about this? Is it possible to have surgery again?

mafan, 30~39 year old female. Ask Date: 2007/01/22

Hello: After undergoing corrective surgery for congenital penile curvature, it seems that the sutures of the tunica albuginea may have ruptured.
It is important to monitor for any signs of hematoma and whether there is any deformity of the penis.
It is advisable to seek medical attention.
Whether another surgery is necessary will be determined after an examination.
Sincerely, Dr.

Managing penile curvature post-surgery can be a complex issue, especially when complications arise such as pain and changes in curvature after a corrective procedure. It’s essential to approach this situation with care and seek appropriate medical advice. Here’s a comprehensive overview of your concerns and potential next steps.

Understanding Your Condition
After undergoing surgery for penile curvature, it is not uncommon to experience some discomfort or pain, especially if there has been trauma to the surgical site. The fact that you mentioned a possible rupture of the sutures in the corpus cavernosum (the erectile tissue) indicates that there may be a need for further evaluation. The pain you are experiencing during erections could be due to several factors, including:
1. Suture Rupture: If the sutures have indeed ruptured, it could lead to instability in the structural integrity of the penis, causing pain and changes in curvature.

2. Scar Tissue Formation: Post-surgical healing can lead to the formation of scar tissue, which may affect the elasticity and shape of the penis.

3. Inflammation: Any trauma to the area can cause inflammation, leading to pain and discomfort during erections.

Immediate Steps to Take
1. Consult Your Surgeon: The first and most crucial step is to contact the surgeon who performed your initial procedure. They will have the best understanding of your surgical history and can assess the current state of your condition. They may recommend imaging studies, such as an ultrasound, to evaluate the integrity of the tissues and sutures.

2. Avoid Further Trauma: Until you have been evaluated, it is vital to avoid any activities that could exacerbate the situation, including sexual activity or any vigorous physical activities that could put strain on the area.

3. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help manage your pain. However, consult with your healthcare provider before taking any medication, especially if you have other underlying health conditions or are taking other medications.

Potential Treatment Options
1. Surgical Revision: If the evaluation confirms that the sutures have ruptured and the curvature has returned, a surgical revision may be necessary. This could involve re-suturing the affected area or possibly performing a different type of corrective procedure, depending on the extent of the issue.

2. Physical Therapy: In some cases, pelvic floor physical therapy may help improve function and reduce pain. A physical therapist specializing in men's health can provide exercises that may assist in recovery.

3. Medication: If inflammation is a significant issue, your doctor may prescribe anti-inflammatory medications or other treatments to help reduce pain and swelling.

4. Counseling: If the psychological impact of your condition is significant, consider speaking with a mental health professional. They can help you cope with any anxiety or distress related to your condition.

Conclusion
In summary, it is crucial to address your concerns with a qualified healthcare provider who can assess your situation accurately. Do not hesitate to seek a second opinion if you feel it is necessary. Remember that recovery from surgery can take time, and it’s essential to be patient with the healing process. Your health and well-being are paramount, so prioritize getting the appropriate care and support.
