Urological Issues: Itchy Symptoms and Possible Infections - Urology

Hello!

Dear Doctor, I need your help to address an embarrassing issue.
At the beginning of February, I noticed that my scalp was itchy and there was more dirt than usual.
For example, after not showering for three days, I would find a white ring around my scalp.
However, I noticed that it only took one day for it to accumulate that much.
After a couple of days, the itching became unbearable, so I went to see a doctor.
The doctor only prescribed me an anti-itch cream.
After 5 to 6 days, I was still very itchy and developed white spots, so I went back to the doctor, who again prescribed the same anti-itch cream.

The next day, I noticed that the white spots on my glans had turned into pustules.
When I squeezed them, a milky white discharge came out, and the itching was unbearable and painful.
Could you please tell me what kind of sexually transmitted infection I might have? (I hadn’t been intimate with my girlfriend for three weeks before the symptoms appeared, and she is very healthy with no signs of infection.) I suspect that I might have touched something dirty and then touched my genital area, which led to this issue.
I would like to get tested for STIs, including blood tests, swabs, and bacterial cultures, to determine what I might be infected with so that I can receive appropriate treatment.
Please give me some advice.
Thank you!

It is truly unfortunate.
Based on your description, it is highly likely that you are experiencing a fungal infection and inflammation of the glans and foreskin.
I recommend that you visit a hospital to consult with a urologist for examination and treatment.

Understanding urological issues, particularly those involving itchy symptoms and possible infections, is crucial for effective diagnosis and treatment. Based on your description, it seems you are experiencing a series of symptoms that could indicate a dermatological or infectious condition affecting the genital area.

The symptoms you mentioned, such as itching, the presence of white discharge, and the development of pustules, are concerning and warrant a thorough evaluation. It is important to note that while some of these symptoms may suggest a sexually transmitted infection (STI), they could also be indicative of other conditions, such as a fungal infection, bacterial infection, or even a skin condition like dermatitis.

1. Itching and Discharge: The initial itching and the presence of a white substance could be due to a variety of causes. Commonly, conditions like candidiasis (a yeast infection) or bacterial infections can lead to similar symptoms. In men, a yeast infection can cause itching and a white discharge, often mistaken for an STI.
2. Pustules and Pain: The development of pustules that are painful and discharge a white fluid is particularly concerning. This could indicate a bacterial infection, such as folliculitis or a sexually transmitted infection like herpes or gonorrhea. The fact that you mentioned squeezing the pustule and it releasing a creamy fluid suggests that there may be an infection present that requires medical attention.

3. Importance of Testing: Given your symptoms, it is essential to undergo appropriate testing. This may include:
- STI Screening: Blood tests and swabs can help identify infections such as chlamydia, gonorrhea, and herpes.

- Culture Tests: A culture of the discharge can help identify the specific bacteria or fungus causing the infection.

- Urinalysis: This can help rule out urinary tract infections (UTIs) that may also cause similar symptoms.

4. Consultation with a Specialist: If your symptoms persist despite using topical treatments, it is advisable to consult a urologist or a dermatologist who specializes in genital conditions. They can provide a more comprehensive evaluation and may recommend treatments that are more effective than over-the-counter options.

5. Hygiene and Care: In the meantime, maintaining good hygiene is crucial. Ensure that the genital area is kept clean and dry, and avoid using harsh soaps or products that may irritate the skin. Wearing breathable, cotton underwear can also help reduce moisture and irritation.

6. Avoid Self-Diagnosis: While it is natural to speculate about the cause of your symptoms, self-diagnosis can lead to unnecessary anxiety and may delay appropriate treatment. It is best to rely on professional medical advice.

In conclusion, your symptoms indicate that you may be dealing with an infection or another dermatological issue that requires proper diagnosis and treatment. It is essential to follow up with a healthcare provider who can perform the necessary tests and provide you with a tailored treatment plan. Do not hesitate to seek medical attention, as early intervention can lead to better outcomes and relief from your symptoms.
