Which specialty should I see for upper back pain?

Hello: Recently, I have been experiencing pain in my upper left back every day.
By the afternoon, or after sitting for a long time, I also feel pain from my lower back down to my thighs.
Even using pain relief patches does not seem to help.
I'm not sure which medical specialty I should see for an examination.
Thank you.

Hello: Typically, such issues should be addressed by an orthopedic specialist.
However, based on your age, it seems to be work-related.
I recommend that you take a few days off to see if it dramatically improves.
If resting does not lead to any improvement, then you should consult a physician.
Thank you for your inquiry, and I hope the above information helps to resolve your concerns.

When dealing with upper back pain, especially when it radiates to the lower back and thighs, it is essential to consult the right specialist to ensure proper diagnosis and treatment. Given your symptoms, you may want to consider seeing a few different types of healthcare providers.

1. Primary Care Physician (PCP): Your first step should be to visit your primary care physician. They can perform an initial evaluation, take a detailed medical history, and conduct a physical examination. Your PCP can help rule out any serious underlying conditions and may refer you to a specialist if necessary.

2. Orthopedic Specialist: If your PCP suspects that your pain may be related to musculoskeletal issues, they might refer you to an orthopedic specialist. Orthopedic doctors specialize in the diagnosis and treatment of musculoskeletal disorders, including issues related to bones, joints, ligaments, tendons, and muscles. They can order imaging tests such as X-rays or MRIs to identify any structural problems.

3. Physical Medicine and Rehabilitation (PM&R) Specialist: Also known as physiatrists, these specialists focus on restoring function and improving quality of life for patients with musculoskeletal issues. They can provide a comprehensive rehabilitation plan that may include physical therapy, pain management techniques, and other non-surgical treatments.

4. Physical Therapist: If your pain is determined to be muscular or related to posture, a physical therapist can be very beneficial. They can develop a personalized exercise program to strengthen the muscles in your back, improve flexibility, and teach you proper body mechanics to prevent further injury.

5. Chiropractor: Some patients find relief from upper back pain through chiropractic care. Chiropractors focus on spinal alignment and may use manual manipulation to relieve pain and improve function. However, it's essential to ensure that your condition is suitable for chiropractic treatment.

6. Rheumatologist: If there is a suspicion of an inflammatory condition or autoimmune disorder contributing to your pain, a rheumatologist may be consulted. They specialize in diagnosing and treating conditions that affect the joints and connective tissues.

7. Pain Management Specialist: If your pain is chronic and not responding to standard treatments, a pain management specialist may be helpful. They can offer various interventions, including medications, injections, and other advanced pain management techniques.

In summary, the best course of action is to start with your primary care physician, who can guide you through the process of diagnosis and treatment. They will consider your symptoms, medical history, and any necessary tests to determine the most appropriate specialist for your upper back pain. Additionally, it’s crucial to communicate openly about your symptoms, including when the pain occurs, its intensity, and any factors that exacerbate or relieve it. This information will help your healthcare provider make an informed decision regarding your care.
Remember, early intervention is key to preventing chronic pain and ensuring a quicker recovery, so don’t hesitate to seek medical attention for your symptoms.
