Urinary Urgency and Prostate Fluid Release: Common Concerns - Urology

How could this happen?

Hello, doctor.
I would like to ask a few questions:
1.
Recently, I've noticed that sometimes when I urgently need to urinate, I hold it in, but after I finish urinating, I experience a sensation similar to ejaculation, and suddenly I feel fluid flowing from my urethra, which I realize is prostatic fluid.
Why does this happen?
2.
I rarely masturbate, but I have a high frequency of nocturnal emissions, and the volume of semen during these emissions is quite significant.
Why is this the case? Should I consider regular masturbation to relieve this?
3.
Could it be related to the fact that I rarely masturbate? I also engage in daily activities, but I often feel a strong urge to ejaculate when my penis is erect.
Is there a connection?
Thank you for your assistance, doctor.

Dr. Xu Weikai reply Urology

Hello! 1.
If the seminal vesicles are full, some individuals may experience a slight leakage of semen during urination.
2.
Nocturnal emissions are completely normal.
Whether or not to engage in self-management is a personal decision, as nocturnal emissions do not affect health.
3.
Reduced ejaculation can lead to a feeling of fullness in the seminal vesicles, which may create a desire to ejaculate during an erection.
Response from Dr.

Understanding urinary urgency and prostate fluid release can be complex, especially when considering the interplay between the urinary and reproductive systems. Let's break down your concerns one by one.

1. Urinary Urgency and Prostate Fluid Release: The sensation of needing to urinate urgently, followed by the feeling of wanting to ejaculate, can be attributed to the close anatomical relationship between the bladder and the prostate. The prostate gland produces prostatic fluid, which is a component of semen. When the bladder fills and the urge to urinate increases, it can stimulate the prostate, leading to the release of prostatic fluid. This is not uncommon and can happen even in the absence of ejaculation. The prostate and bladder share nerve pathways, which can sometimes cause confusion in sensations. If you are experiencing this frequently or if it is accompanied by pain or discomfort, it would be wise to consult a urologist for a thorough evaluation.

2. Dream Erection and Semen Release: The phenomenon of nocturnal emissions, commonly known as "wet dreams," is a normal part of male physiology, especially in younger men or those who may not ejaculate frequently through masturbation or sexual intercourse. The body has a natural way of releasing built-up semen, and this can occur during sleep, often accompanied by erotic dreams. The amount of semen released can vary based on several factors, including frequency of ejaculation and individual physiological differences. If you find that you are experiencing frequent nocturnal emissions, it may indicate that your body is responding to a buildup of sexual tension. Regular ejaculation, whether through masturbation or sexual activity, can help manage this buildup, but it is not strictly necessary unless you feel discomfort.

3. Erection and Sexual Urges: The feelings of arousal and the urge to ejaculate when experiencing an erection are normal physiological responses. The body is designed to respond to sexual stimuli, and if you are not regularly ejaculating, this can lead to increased sexual tension. Engaging in regular sexual activity or masturbation can help alleviate this tension and may reduce the frequency of unwanted erections or urges. However, if you find that these feelings are interfering with your daily life or causing distress, it may be beneficial to speak with a healthcare provider or a mental health professional who specializes in sexual health.

In summary, the sensations you are experiencing are likely related to the normal functioning of your urinary and reproductive systems. However, if you have concerns about the frequency or nature of these experiences, or if they are accompanied by pain or other symptoms, it is important to seek medical advice. Regular sexual activity or masturbation can help manage sexual tension, but it is a personal choice and should be based on your comfort and lifestyle. Always feel free to discuss any concerns with your healthcare provider to ensure that you receive personalized advice tailored to your health needs.
