Questions regarding ejaculation?

I am 29 years old and unmarried, and I have a question regarding ejaculation.
Recently, I have noticed that after using the restroom, I feel like I haven't completely emptied my bladder, and I immediately feel the urge to urinate again, only to pass a small amount.
I've also experienced a few instances where just before ejaculation, I feel the need to urinate, so I go to the restroom first and then ejaculate shortly after.
Additionally, there was an occasion when I noticed that my semen seemed to be mixed with urine during ejaculation.
I read that urination and ejaculation should not occur simultaneously, and that ejaculation should take precedence.
Should I seek medical evaluation? If I go to the hospital, what kind of examinations should I undergo?
P.S.
Based on my observations, when I masturbate with my hand, I do not experience these issues, but when I use a masturbation sleeve, the problems arise.
