Can weight loss cause missed periods?
Hello, Doctor Dai: I have lost 12 kilograms over the course of more than 80 days, going from 67 kg to 55 kg.
My weight loss method includes having a normal breakfast from outside vendors (both Western and Chinese styles), a lunch consisting of steamed vegetables, a small portion of starches, and a little protein from eggs, beans, fish, and meat (mostly boiled), and for dinner, I only eat two types of fruit, sometimes adding soda crackers.
In addition to dietary control, I exercise daily, walking briskly for one hour.
My menstrual cycle was normal in May, but in June it only lasted for two days and was dark in color, and I haven't had my period since July, with no signs of it in August either.
I would like to know if this is due to the rapid weight loss, and how I should improve this situation.
Should I see a gynecologist or an endocrinologist?

Rapid weight loss can indeed lead to missed periods, a condition known as amenorrhea. This phenomenon is often linked to significant changes in body weight, particularly when the weight loss is rapid and substantial, as in your case. When the body experiences drastic changes in weight, it can affect the hormonal balance that regulates the menstrual cycle.
The menstrual cycle is primarily controlled by hormones such as estrogen and progesterone, which are produced by the ovaries. When you lose weight quickly, your body may interpret this as a form of stress, leading to alterations in the hypothalamus, the part of the brain responsible for regulating hormones. This can result in decreased production of gonadotropin-releasing hormone (GnRH), which in turn reduces the secretion of luteinizing hormone (LH) and follicle-stimulating hormone (FSH) from the pituitary gland. These hormones are crucial for ovulation and menstruation.
In your situation, the rapid weight loss from 67 kg to 55 kg over a short period, combined with a restrictive diet and increased physical activity, could have contributed to the hormonal imbalance leading to your missed periods. The fact that your menstrual cycle was normal in May but became irregular in June and then stopped in July suggests that the changes in your body due to weight loss and dietary changes are likely influencing your menstrual health.
To address this issue, it is essential to consider a few steps:
1. Consult a Healthcare Professional: It would be wise to see a gynecologist or an endocrinologist who specializes in metabolic disorders. They can evaluate your hormonal levels and overall health to determine the underlying cause of your missed periods.
2. Nutritional Assessment: A registered dietitian can help you develop a balanced eating plan that supports healthy weight loss without being overly restrictive. This plan should ensure you are getting adequate nutrients, including healthy fats, proteins, and carbohydrates, which are essential for hormonal balance.
3. Gradual Weight Loss: If you are still aiming to lose weight, consider adopting a more gradual approach. Aim for a weight loss of about 0.5 to 1 kg per week, which is generally considered safe and sustainable. This slower pace can help your body adjust without triggering hormonal disruptions.
4. Monitor Physical Activity: While exercise is beneficial for weight management and overall health, excessive physical activity can also lead to missed periods. Ensure that your exercise routine is balanced and not overly strenuous. Incorporating rest days and varying your workouts can help.
5. Stress Management: Stress can also impact your menstrual cycle. Consider incorporating stress-reducing activities such as yoga, meditation, or mindfulness practices into your routine.
6. Regular Check-ups: Keep track of your menstrual cycle and any other symptoms you may experience. Regular check-ups with your healthcare provider can help monitor your progress and make necessary adjustments to your plan.
In summary, rapid weight loss can indeed cause missed periods due to hormonal imbalances. It is crucial to approach weight loss in a healthy, sustainable manner and seek professional guidance to ensure that your overall health is prioritized. By addressing your dietary habits, exercise routine, and stress levels, you can work towards restoring your menstrual cycle and maintaining a healthy weight.
