Struggling with Digestive Issues: Seeking Relief for Gut Health - Gastroenterology and Hepatology

Hello doctor, please help me with my gastrointestinal issues?

Hello, doctor.
To begin with, I have regular bowel movements every morning, so I don't have issues with constipation.
However, I have a sensitive gastrointestinal system and I am lactose intolerant, which makes me prone to diarrhea when consuming dairy products.
I often see advertisements claiming that drinking yogurt can improve gut health and increase beneficial bacteria, but ironically, I have experienced diarrhea even from yogurt (although it's not every time, it has happened).
Additionally, I find that eating greasy foods also sends me running to the bathroom, and eating too quickly can cause issues as well.
I rarely get sick and see a doctor, but my gastrointestinal problems (such as irritable bowel syndrome) have led me to seek medical attention multiple times.
Once, I went to a military hospital for a comprehensive examination (except for an endoscopy), and no issues were found.
The doctor concluded that I simply have poor gastrointestinal function.
How can I improve my gut function? I want to drink yogurt, but I'm afraid of diarrhea.
This is really troubling, especially since I can't consume it with food; I must drink it between meals because drinking it with food significantly increases the likelihood of diarrhea.
Please help me; I don't want to continue like this.
Thank you very much.

Experiencing diarrhea after eating can be quite troubling.
Lactose intolerance is different from irritable bowel syndrome (IBS), and it is important to avoid dairy products in the case of lactose intolerance.
IBS is related to emotions, infections, and diet.
It is recommended to maintain a positive mood, avoid excessive stress, and limit the intake of irritating foods and beverages (such as coffee and chocolate).
Developing good eating habits, such as eating regular meals and chewing slowly, is also advised.
If diarrhea persists, consider the short-term use of probiotics.
Wishing you good health, Dr.

Digestive issues can be quite challenging, especially when they are compounded by conditions like lactose intolerance and irritable bowel syndrome (IBS). It sounds like you have been experiencing a range of gastrointestinal symptoms that are affecting your quality of life. Here are some insights and recommendations that may help you manage your gut health more effectively.

Understanding Your Symptoms
1. Lactose Intolerance: Since you mentioned that dairy products, including yogurt, can lead to diarrhea, it’s essential to recognize that lactose intolerance is a common condition where the body lacks the enzyme lactase, which is necessary to digest lactose, the sugar found in milk. This can lead to symptoms such as bloating, gas, and diarrhea after consuming dairy.

2. Irritable Bowel Syndrome (IBS): IBS is a functional gastrointestinal disorder characterized by symptoms like abdominal pain, bloating, and changes in bowel habits. Stress, diet, and gut flora can all play significant roles in exacerbating IBS symptoms.

Recommendations for Improving Gut Health
1. Dietary Adjustments:
- Lactose-Free Options: Since you experience issues with dairy, consider switching to lactose-free yogurt or plant-based alternatives like almond or coconut yogurt. These options can provide beneficial probiotics without triggering your symptoms.

- Fiber Intake: Incorporate soluble fiber into your diet, which can help regulate bowel movements. Foods like oats, bananas, and apples are good sources. However, be cautious with insoluble fiber (found in whole grains and certain vegetables) as it can sometimes worsen IBS symptoms.

- Avoid Trigger Foods: Keep a food diary to identify specific foods that trigger your symptoms. Common culprits include high-fat foods, spicy foods, and certain artificial sweeteners.
2. Probiotics: While you have concerns about yogurt, consider trying different probiotic supplements that are specifically designed for gut health. Look for products that contain strains like Lactobacillus and Bifidobacterium, which may help improve gut flora without causing digestive upset.

3. Eating Habits:
- Mindful Eating: Eating too quickly can lead to swallowing air and increased gastrointestinal discomfort. Try to eat slowly, chew your food thoroughly, and take breaks between bites.

- Regular Meal Times: Establish a routine for your meals. Eating at consistent times can help regulate your digestive system.

4. Hydration: Ensure you are drinking enough water throughout the day. Staying hydrated is crucial for digestive health and can help prevent constipation.

5. Stress Management: Since stress can exacerbate IBS symptoms, consider incorporating relaxation techniques into your daily routine. Practices such as yoga, meditation, or deep-breathing exercises can be beneficial.

6. Consult a Specialist: If your symptoms persist despite making these changes, it may be worthwhile to consult a gastroenterologist or a registered dietitian who specializes in digestive health. They can provide personalized advice and may suggest further testing to rule out other conditions.

Conclusion
Managing digestive issues, especially with conditions like lactose intolerance and IBS, requires a multifaceted approach. By making dietary adjustments, being mindful of your eating habits, and managing stress, you can improve your gut health and overall well-being. Remember, it’s essential to listen to your body and make changes that feel right for you. If you continue to experience significant discomfort, seeking professional guidance is always a wise choice.
