There are white spots on the foreskin?

Hello, doctor: I have had sexual intercourse with my boyfriend (23 years old, we are both monogamous partners), and I noticed some small white spots under his foreskin.
Later, I also found similar small white spots inside the corners of his mouth and underneath his tongue.
I'm unsure if these are related.
Those small white spots seem to have been there for a long time and haven't changed or caused any issues, but I am still a bit worried.
Could this be a sign of a disease?

Understanding the presence of white spots on the foreskin and other areas of the mouth can be concerning, especially when they appear in multiple locations. In your case, the small white spots on your boyfriend's foreskin, as well as similar spots in his mouth, could be indicative of several conditions, but it's important to approach this with a clear understanding of potential causes and their implications.

Firstly, the white spots on the foreskin could be related to a benign condition known as Fordyce spots. These are small, painless, pale bumps that are actually sebaceous glands that are not associated with hair follicles. They are completely normal and harmless, often appearing on the lips and genital areas. If these spots have been present for a long time without any changes, it is likely that they are Fordyce spots.

Another possibility is the presence of pearly penile papules, which are small, dome-shaped bumps that appear around the corona of the glans penis. These are also benign and do not require treatment. They are a normal anatomical variant and are not associated with any disease or infection.

On the other hand, if the white spots in the mouth are also present, they could be related to oral conditions such as oral thrush (a fungal infection), leukoplakia (white patches that can occur due to irritation or other factors), or even oral lichen planus. Oral thrush is characterized by creamy white lesions that can be painful and may indicate an overgrowth of Candida fungus, especially if there are risk factors such as immunosuppression or antibiotic use. Leukoplakia, on the other hand, can be a response to chronic irritation, and while it is often benign, it can sometimes be precancerous, so it should be monitored.

Given that the spots have not changed over time and do not cause any discomfort, it is less likely that they are indicative of a serious condition. However, the fact that they are present in both the genital and oral areas could suggest a need for further evaluation, especially if there are any accompanying symptoms such as pain, itching, or changes in appearance.

It is crucial to consult a healthcare professional for an accurate diagnosis. A doctor can perform a physical examination and may recommend tests if necessary to rule out any infections or other conditions. They can also provide reassurance and guidance on whether any treatment is needed.

In summary, while the presence of white spots on the foreskin and in the mouth can be concerning, many of these conditions are benign and not indicative of serious health issues. However, due to the potential for various underlying causes, it is advisable to seek medical advice for a thorough evaluation and peace of mind. Regular check-ups and open communication with healthcare providers are essential for maintaining sexual health and addressing any concerns that may arise.
