Dear Dr.
Liu,
Subject: Recently noticed a white substance on the glans during bathing.
I am an individual who has not undergone circumcision.
Every day during my shower, I make sure to retract the foreskin to clean the glans.
However, for the past week or so, I have been experiencing a cold due to a recent illness, for which I consulted an internist and received medication.
During my shower, I noticed a white substance on the glans, and the inner side of the foreskin was itchy.
After cleaning, the white substance was removed, and the itching subsided after a while.
This has been ongoing for about a week.
Fortunately, the itching has stopped in the last couple of days.
Could you please advise on how to manage this symptom? Thank you for your time.

That's great! You're handling it well.
Daily cleaning is a good habit.
As for the itching symptoms, it may be due to a cold, low physical strength, and reduced immunity, which can lead to a fungal infection caused by moisture under the foreskin.
If it recurs, you should visit a urologist for an examination and prescription of a topical ointment.

Understanding white discharge on the glans can be concerning, especially for individuals who have not undergone circumcision. The symptoms you describe, including the presence of a white substance on the glans and itching of the inner foreskin, are not uncommon and can be attributed to several potential causes.

Firstly, the white discharge you observed could be a result of smegma, which is a natural substance made up of dead skin cells, oils, and other bodily secretions that can accumulate under the foreskin. Smegma is typically harmless but can lead to irritation and discomfort if not cleaned regularly. Since you mentioned that you wash the area daily, it’s important to ensure that you are cleaning thoroughly but gently to avoid irritation.

Another possible cause of the white discharge could be a yeast infection, particularly if you have experienced any changes in your hygiene routine or if you have been on antibiotics for your recent illness. Yeast infections can cause symptoms such as itching, irritation, and a white discharge that may resemble cottage cheese. If this is the case, antifungal treatments may be necessary, and it would be advisable to consult a healthcare provider for an accurate diagnosis and appropriate treatment.

Additionally, sexually transmitted infections (STIs) could also present with similar symptoms, although they are often accompanied by other signs such as pain during urination or unusual discharge. If you are sexually active, it’s important to consider this possibility and seek testing if necessary.

Given that the itching has subsided and the white substance washes away easily, it may indicate that the issue was temporary and related to irritation or mild infection. However, if symptoms persist or recur, it is crucial to seek medical advice. A healthcare provider can perform a physical examination and may recommend tests to rule out any infections or other underlying conditions.

In terms of management, here are some general recommendations:
1. Maintain Good Hygiene: Continue to wash the area daily with mild soap and water. Ensure that you rinse thoroughly and dry the area well to prevent moisture buildup, which can promote fungal growth.

2. Avoid Irritants: Be cautious with soaps, lotions, or other products that may irritate the sensitive skin of the genital area. Opt for unscented and hypoallergenic products.

3. Monitor Symptoms: Keep an eye on any changes in symptoms. If you notice increased redness, swelling, persistent itching, or any unusual discharge, it’s important to seek medical attention.

4. Consult a Healthcare Provider: If the white discharge returns or if you experience any new symptoms, consult a healthcare provider. They may perform a physical examination and recommend appropriate tests or treatments based on their findings.

In conclusion, while the presence of a white discharge on the glans can be concerning, it is often manageable with proper hygiene and care. However, persistent or severe symptoms warrant a visit to a healthcare professional for further evaluation and treatment.
