Nasal mucus with blood?

I would like to consult Dr.
Yeh regarding my habit of picking my nose, as I sometimes notice that the mucus I extract is blood-stained.
This has been happening intermittently for a while now.
I have researched various potential causes and would like to know what the most likely reason might be.
I have a history of nasal allergies from childhood, and my doctor mentioned that my nasal mucosa is thinner.
Could this be the reason? Please help clarify this for me.
Thank you.

Xiao Yu, 20~29 year old female. Ask Date: 2009/07/25

Dr. Ye Dawei reply Otolaryngology

Dear Sir/Madam: The most likely reason is this; you may undergo a nasopharyngoscopy at the outpatient clinic.

Yeh Da-Wei, Head of the ENT Department, Hsinchu Hospital.

When you notice blood in your nasal mucus, it can be concerning, and understanding the underlying causes is essential for addressing the issue effectively. There are several common reasons why this might occur, particularly in individuals with a history of nasal allergies or thin nasal mucosa, as you mentioned.

1. Nasal Mucosa Fragility: As you noted, having a history of nasal allergies can lead to chronic inflammation of the nasal mucosa. This inflammation can cause the blood vessels in the nasal lining to become fragile and more susceptible to rupture, especially when you engage in activities like nose picking. The act of digging into the nose can easily damage these delicate blood vessels, leading to bleeding.

2. Dry Air: Environmental factors play a significant role in nasal health. Dry air, particularly during winter months or in arid climates, can dry out the nasal mucosa, making it more prone to cracking and bleeding. If you live in an area with low humidity or use heating systems that dry out the air, this could exacerbate the problem.

3. Nasal Trauma: Frequent nose picking or aggressive blowing of the nose can lead to minor injuries in the nasal passages. These injuries can result in bleeding, especially if the mucosa is already compromised due to allergies or dryness.

4. Infections: Conditions such as sinusitis or upper respiratory infections can also lead to blood in nasal mucus. Infections can cause inflammation and irritation of the nasal passages, leading to increased vascularity and potential bleeding.

5. Structural Issues: Anatomical abnormalities such as a deviated septum can contribute to uneven airflow and pressure in the nasal passages, which may lead to localized irritation and bleeding.

6. Underlying Health Conditions: In rare cases, more serious conditions such as bleeding disorders or tumors (like nasal polyps or, although less common, nasal cancers) can cause blood in the nasal mucus. However, these are less likely, especially if you have no other concerning symptoms.

Given your history of nasal allergies and the thinness of your nasal mucosa, it is quite plausible that these factors are contributing to the occasional presence of blood in your nasal mucus.

Recommendations:
- Avoid Nose Picking: Try to refrain from picking your nose, as this can exacerbate the problem by causing trauma to the already fragile mucosa.

- Humidify Your Environment: Using a humidifier in your home can help maintain moisture in the air, reducing the likelihood of your nasal passages drying out.

- Saline Nasal Sprays: Regular use of saline sprays can help keep your nasal passages moist and may reduce the occurrence of bleeding.

- Consult an ENT Specialist: If the problem persists or worsens, it would be wise to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination and may recommend treatments such as topical medications or further investigations if necessary.

In summary, while occasional blood in nasal mucus can be alarming, it is often due to benign causes such as nasal trauma, dryness, or inflammation. By taking preventive measures and seeking medical advice when needed, you can manage this condition effectively.
