Children may experience lower back pain when urinating?

Hello Doctor: My child complained of back pain while urinating today, and it took her three attempts to finish urinating.
Upon further questioning, I found out that she did not use the restroom at school all day, nor did she at her after-school care.
She went from 7 AM until 8 PM without using the bathroom, and this has been happening for a while.
I would like to know if these symptoms are serious, what should I pay attention to, and whether we should see a nephrologist or a urologist? Thank you.

Dr. Pei Rensheng reply Pediatrics

Dear Maggie: Hello, the symptoms in your child strongly suggest a urinary tract infection.
Please ensure they drink plenty of water and urinate frequently.
It is important to consult a pediatrician.
Generally, the physician will perform a urinalysis, and if a urinary tract infection is confirmed, antibiotic treatment will be necessary.
If there is also kidney inflammation, hospitalization may be required, so please do not take this lightly.
Wishing you health and happiness! Sincerely, Dr.
Pei Ren-Sheng, Chief of Pediatrics, Taoyuan Hospital, Department of Health.

More Info

When a child experiences back pain while urinating, it can be concerning for parents. In your case, the child has reported pain in the lower back during urination and has also mentioned that she has not urinated throughout the day while at school and after-school care. This situation raises several important points that parents should consider.

Understanding the Symptoms
1. Frequency of Urination: The fact that your child has not urinated for an extended period (from 7 AM to 8 PM) is significant. Children should be encouraged to urinate regularly, and not doing so can lead to discomfort and potential complications, such as urinary tract infections (UTIs) or bladder distension.

2. Back Pain: Pain in the lower back during urination can indicate several issues. It could be related to the bladder or kidneys, especially if there is a urinary tract infection or other underlying conditions affecting the urinary system.
3. Potential Causes:
- Urinary Tract Infection (UTI): UTIs are common in children and can cause pain during urination, frequent urination, and back pain.
- Bladder Issues: Holding urine for too long can lead to bladder distension and discomfort.

- Kidney Problems: Although less common, kidney issues can also manifest as back pain and urinary symptoms.

Recommended Actions
1. Encourage Regular Bathroom Breaks: It is crucial to encourage your child to use the bathroom regularly, especially during school hours. Children should be taught the importance of not holding in urine for extended periods.

2. Monitor Symptoms: Keep an eye on any additional symptoms that may arise, such as fever, changes in urine color, foul-smelling urine, or increased pain. These could indicate a more serious issue that requires medical attention.

3. Consult a Healthcare Professional: Given the symptoms described, it is advisable to consult a healthcare provider. In this case, a pediatrician would be the first point of contact. They can perform an initial evaluation and determine whether a referral to a specialist, such as a nephrologist (kidney specialist) or urologist (urinary tract specialist), is necessary.

4. Diagnostic Tests: The doctor may recommend urine tests to check for signs of infection, blood, or other abnormalities. They may also suggest imaging studies if there are concerns about the kidneys or bladder.

Conclusion
In summary, your child's symptoms of back pain while urinating, combined with the history of not urinating throughout the day, warrant attention. It is essential to encourage regular bathroom use and consult a pediatrician for a thorough evaluation. Early intervention can help prevent complications and ensure your child receives the appropriate care. Remember, addressing these issues promptly can lead to better outcomes and alleviate any discomfort your child may be experiencing.
