The issue of smegma accumulation?
Hello, Doctor Hsu.
I have a problem with phimosis (excessive foreskin).
A few months ago, I used baby oil for lubrication while masturbating, and I might not have washed it off before going to bed.
Later, I noticed that the color of my glans turned a bit whitish and it won't wash off.
Additionally, there is smegma between the coronal sulcus and the foreskin, which has caused the glans and foreskin to stick together.
The foreskin itself also seems to be somewhat adhered.
I have since stopped using baby oil, but those white substances won't come off no matter what I do.
A more serious issue is that due to the adhesions, I experience pain when my penis is erect.
What should I do? Besides water, are there any other solutions or solvents I can use for cleaning? Thank you very much!

You can use soap or some cleaning agents to dissolve baby oil.
Additionally, you can use a towel to scrub directly, which can effectively remove it.

Dealing with smegma and phimosis can be uncomfortable and concerning, especially when it leads to pain and other symptoms. Smegma is a natural substance that can accumulate under the foreskin, consisting of dead skin cells, oils, and other bodily fluids. When combined with phimosis, which is the inability to retract the foreskin fully, it can lead to inflammation, pain, and potential infections.
First and foremost, it’s essential to maintain proper hygiene. Regular cleaning of the genital area is crucial, especially for those with phimosis. Here are some steps you can take to manage your symptoms:
1. Gentle Cleaning: Use warm water to gently clean the area under the foreskin. Avoid using harsh soaps or chemicals, as these can irritate the sensitive skin. If you find that smegma is persistent, you may consider using a mild, unscented soap, but ensure that it is thoroughly rinsed off.
2. Avoid Irritants: Since you mentioned using baby oil, it’s wise to avoid any products that can cause irritation or allergic reactions. Stick to water for cleaning, and if you need lubrication for sexual activities, consider water-based lubricants that are designed for sensitive skin.
3. Consult a Healthcare Provider: Given that you are experiencing pain during erections and that the foreskin seems to be stuck, it’s crucial to consult a healthcare provider, preferably a urologist. They can assess the severity of your phimosis and recommend appropriate treatments. In some cases, a topical steroid cream may be prescribed to help loosen the foreskin, or in more severe cases, a minor surgical procedure may be necessary.
4. Topical Treatments: If there is inflammation or infection, your doctor may prescribe topical antifungal or antibacterial creams. These can help reduce any irritation caused by smegma buildup or potential infections.
5. Stretching Exercises: If your doctor approves, gentle stretching of the foreskin can sometimes help alleviate phimosis. This should be done carefully and gradually to avoid pain or injury.
6. Avoiding Sexual Activity: Until the issue is resolved, it may be wise to avoid sexual activity, especially if it causes pain. Engaging in sexual activities with phimosis can lead to further complications, including tearing of the foreskin or increased pain.
7. Surgical Options: If conservative treatments do not alleviate the symptoms, surgical options such as circumcision or preputioplasty may be considered. These procedures can effectively resolve phimosis and prevent future issues with smegma buildup.
In summary, while smegma and phimosis can be uncomfortable and lead to painful symptoms, maintaining proper hygiene and consulting with a healthcare professional are essential steps in managing the condition. Avoid using irritants, and seek medical advice for appropriate treatments tailored to your specific situation. Remember, it’s important to address these issues promptly to prevent complications and improve your overall comfort and health.
