The urethral opening feels strange, and white fluid appears when straining during bowel movements?
Hello Doctor: Recently, I've been feeling something unusual with my urethra...
there's a bit of a stinging discomfort.
Sometimes, when I strain during bowel movements, a drop of a substance resembling semen (with a fishy odor) comes out.
The first occurrence was around early January (after being intimate with my girlfriend without engaging in sexual intercourse), and the second time was on February 11.
Therefore, I visited a large hospital to see a doctor on February 12.
After a physical examination, the doctor said there were no signs of inflammation.
I had a urinalysis, and the doctor said everything was normal.
If there is pain, it might be urethritis, so they prescribed medication to treat a bacterial infection for me to take at home.
After taking it for two days, I still felt uncomfortable.
On the morning of February 14, I experienced a small amount of white fluid coming out while straining during a bowel movement.
On February 14, when I was intimate with my girlfriend (due to suspected inflammation, we did not engage in sexual intercourse), I noticed that fluid would leak out during arousal (the amount felt a bit excessive, even soaking through my underwear).
It appeared shiny on the underwear, but after a while, I felt a painful sensation in my bladder and some discomfort in both sides of my lower back.
After a period of calm, I experienced a sensation of residual urine at the urethral opening, feeling like something was trying to push out, with a twitching sensation (about twice).
When I went to the bathroom, the urine felt somewhat similar to semen...
it felt gritty.
Since it's currently the Lunar New Year period, many hospitals are closed, and I'm worried that if it is inflammation, it might delay treatment.
Please help clarify my concerns.
Thank you.

Mr.
Q: Hello, it seems that there is still a sign of infection.
We can start with infection treatment for a while and see how it goes.
Generally, medication can be tried for 7-10 days to improve this condition.
In the meantime, drink plenty of water, and if possible, consume cranberry juice, about 500cc daily.
Once the symptoms improve, you can reduce it to 300cc per day and continue for 14 days.
It might be worth a try.
Note: During the New Year period, most hospitals have outpatient services, so given the convenience of healthcare in Taiwan, you should be able to find a physician.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

Based on the symptoms you've described, it seems you are experiencing a combination of urethral discomfort, unusual discharge, and some associated pain. These symptoms can be concerning, and it's important to understand what they might indicate and how to approach them.
Firstly, the presence of white fluid after straining during bowel movements, especially if it resembles semen and has a fishy odor, could suggest a few potential issues. One possibility is that you may be experiencing a condition known as "pre-ejaculate" fluid, which can sometimes be released during arousal or straining. However, the fact that it has a distinct odor and is associated with discomfort raises the possibility of an underlying infection or inflammation.
Given that you have already seen a doctor and had a urinalysis that came back normal, it’s a good sign that there is no immediate indication of a urinary tract infection (UTI). However, the discomfort you are feeling could still be due to a few other conditions, such as:
1. Urethritis: This is an inflammation of the urethra, which can be caused by infections (bacterial or viral) or irritants. Even if your urinalysis was normal, urethritis can sometimes present with discomfort and discharge without significant findings in a urine test.
2. Prostatitis: Inflammation of the prostate gland can cause discomfort in the urethra, pain during bowel movements, and unusual discharge. This condition can be acute or chronic and may require specific treatment.
3. Pelvic Floor Dysfunction: Straining during bowel movements can sometimes lead to pelvic floor issues, which may cause discomfort in the urethra and a sensation of incomplete emptying.
4. Sexually Transmitted Infections (STIs): Although you mentioned that there were no signs of infection during your visit, some STIs can present with atypical symptoms. It might be worth discussing with your healthcare provider whether further testing for STIs is appropriate.
5. Irritation or Allergic Reaction: Sometimes, irritation from soaps, detergents, or even tight clothing can lead to discomfort and unusual discharge.
In terms of management, here are some steps you can take:
- Hydration: Ensure you are drinking plenty of water. Staying hydrated can help dilute urine and may alleviate some discomfort.
- Avoid Irritants: Steer clear of potential irritants such as scented soaps, lotions, or tight clothing that may exacerbate your symptoms.
- Warm Baths: Taking warm baths can help soothe discomfort in the pelvic area.
- Monitor Symptoms: Keep track of your symptoms, including any changes in discharge, pain levels, or urinary habits. This information can be helpful for your healthcare provider.
- Follow-Up: If symptoms persist or worsen, it’s crucial to follow up with a healthcare provider. Given that you are concerned about the potential for infection, it may be worth seeking care sooner rather than later, especially if you experience fever, increased pain, or changes in urinary habits.
In conclusion, while your initial evaluation did not reveal any significant issues, the persistence of your symptoms warrants further investigation. It’s important to communicate openly with your healthcare provider about your concerns and any changes you notice. Early intervention can prevent complications and lead to more effective treatment.
