Frequent nosebleeds?
I would like to ask about a concern I have.
I often experience nosebleeds without any apparent reason, and it only occurs on the right side.
Each time, the amount of blood is significant, and it can last for 2-3 consecutive days.
I haven't had any trauma, I don't pick my nose, and the weather is neither cold nor dry, yet the bleeding occurs.
Additionally, during these episodes, my blood pressure was recorded at 140/85.
I frequently have nasal congestion, and only after a nosebleed, when I remove the tissue, do I notice a thick, viscous discharge that provides temporary relief.
I previously visited a clinic for examination, but the doctors only performed a visual inspection.
Some mentioned that my blood vessels are too thin, others noted a deviated septum, and one doctor simply said there was a perforation and used a small piece of material to patch it.
Some attributed it to dry and cold weather.
Each doctor provided different explanations, yet the problem persists.
I am concerned that there might be an underlying condition, such as cancer, or if I should undergo further testing.

Hello: In response to your question, if you frequently experience nosebleeds on the same side that are difficult to stop, it is advisable to go to the hospital for a thorough examination.
If possible, I recommend that you go to the hospital immediately when a nosebleed occurs, as this will allow the doctor to clearly see the source of the bleeding and make a more accurate assessment.

More Info
Frequent nosebleeds can be a frustrating and concerning issue, especially when the underlying cause is unclear. Based on your description, it seems that you are experiencing recurrent epistaxis (nosebleeds) primarily from the right nostril, accompanied by nasal congestion and a feeling of pressure relief after the bleeding. The fact that you have had multiple medical opinions without a definitive diagnosis can add to your anxiety about the situation.
Understanding the Causes of Nosebleeds
Nosebleeds can occur for a variety of reasons, and they are generally classified into two categories: anterior and posterior. Anterior nosebleeds, which are the most common, originate from the front part of the nasal cavity and are often due to local factors such as:
1. Dry Air: Even if you feel the weather is not dry, indoor heating can lead to dry nasal mucosa, making it more susceptible to bleeding.
2. Nasal Trauma: This includes not only direct trauma but also frequent nose-picking or blowing the nose forcefully.
3. Allergies or Infections: Conditions that cause nasal congestion can lead to increased pressure in the blood vessels of the nose, resulting in bleeding.
4. Structural Issues: Deviated septum or other anatomical abnormalities can predispose certain areas of the nasal cavity to bleeding.
5. Vascular Conditions: Some individuals have more fragile blood vessels in the nasal lining, which can lead to frequent bleeding.
Posterior nosebleeds are less common but can be more serious, often requiring medical intervention. They usually arise from deeper blood vessels and can be associated with systemic issues.
Potential Medical Concerns
While the majority of nosebleeds are benign, your concern about underlying conditions such as cancer is understandable. However, it is essential to note that frequent nosebleeds are rarely a sign of malignancy. Other serious conditions, such as bleeding disorders (e.g., von Willebrand disease, thrombocytopenia), hypertension, or systemic diseases (e.g., liver disease), can also lead to recurrent nosebleeds.
Recommendations for Management
1. Consult an ENT Specialist: Since you have seen various doctors without a clear diagnosis, it may be beneficial to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, including nasal endoscopy, to visualize the nasal passages and identify any structural abnormalities or sources of bleeding.
2. Blood Tests: Consider asking your doctor for blood tests to evaluate your clotting function and rule out any underlying bleeding disorders. This may include a complete blood count (CBC), prothrombin time (PT), and activated partial thromboplastin time (aPTT).
3. Nasal Care: To help manage and prevent nosebleeds, consider using a saline nasal spray or a humidifier to keep the nasal passages moist. Avoiding irritants such as smoke or strong odors can also be beneficial.
4. Avoidance of Trauma: Be gentle when blowing your nose, and avoid picking at it. If you feel the urge to blow your nose, try to do so gently.
5. Monitor Blood Pressure: Since you mentioned elevated blood pressure readings, it would be wise to monitor your blood pressure regularly and discuss any concerns with your healthcare provider. High blood pressure can contribute to bleeding risks.
6. Lifestyle Modifications: Staying hydrated and maintaining a healthy diet can support overall vascular health. If you smoke, consider quitting, as smoking can irritate the nasal passages.
Conclusion
Frequent nosebleeds can be distressing, but with the right approach, you can find relief and answers. It is crucial to work closely with healthcare professionals who can provide a comprehensive evaluation and tailored management plan. Remember that while it is natural to worry about serious conditions, most nosebleeds are manageable and not indicative of severe underlying diseases. Seeking the right specialist and advocating for thorough investigations will be key in addressing your concerns effectively.
