Missed menstruation?

Hello Doctor, my girlfriend's menstrual dates for the past few months were 2/1, 3/1, and 3/29, all occurring every 28 days.
Her expected period for April should have been on 4/26, but as of today, 5/3, she is a week late.
We had sexual intercourse on 4/17 using a condom, and on 4/18 without a condom but without ejaculation.
On 4/24, we also had intercourse without a condom and without ejaculation.
I calculated the safe period, and both 4/17 and 4/18 were within that range.
Starting the week of 4/19, she began to feel breast tenderness, and on 4/28, she felt like her period was coming, so she used panty liners all day, but it never arrived.
In the past few days, the breast tenderness has also diminished, but her period still hasn't come.
Doctor, could this be a sign of pregnancy? Thank you.

Dr. Dong Yuhong reply Obstetrics and Gynecology

Hello: To determine if you are pregnant, it is more accurate to take a pregnancy test or visit a gynecologist.
Using the safe period for contraception relies on the regularity of your past menstrual cycles to estimate the times when you are less likely to conceive.
However, if your period does not arrive on time, the previously calculated safe period may no longer be reliable.
Therefore, visiting a gynecologist is the best approach, and it is not advisable to rely solely on bodily sensations for judgment.
Thank you.

More Info

When it comes to missed periods and the possibility of pregnancy, it’s essential to consider various factors, including menstrual cycle regularity, timing of sexual activity, and any accompanying symptoms. Based on the information provided, your girlfriend's menstrual cycle has been consistent at approximately 28 days, with her last period expected on April 26. As of May 3, she is now a week late, which understandably raises concerns about potential pregnancy.

Firstly, it’s important to note that while using condoms significantly reduces the risk of pregnancy, no contraceptive method is 100% effective. The fact that there were instances of unprotected intercourse, even without ejaculation, does introduce a slight risk of pregnancy due to pre-ejaculate fluid potentially containing sperm. However, since you mentioned that the sexual activity on April 17 occurred during a supposed safe period, the likelihood of conception is lower, but not impossible.

The symptoms your girlfriend is experiencing, such as breast tenderness (often described as "sore" or "swollen" breasts), can be indicative of hormonal changes that may occur due to various reasons, including the onset of menstruation, pregnancy, or even stress. The feeling of impending menstruation on April 28, followed by the absence of her period, adds to the uncertainty. It’s not uncommon for women to experience premenstrual symptoms without the actual onset of menstruation, especially if there are fluctuations in hormone levels or stress factors involved.

Given that she is now a week late, it would be advisable for her to take a home pregnancy test. These tests are most accurate when taken after a missed period, as they measure the hormone hCG (human chorionic gonadotropin), which is produced during pregnancy. If the test is positive, it would confirm pregnancy, and she should follow up with a healthcare provider for further evaluation and prenatal care. If the test is negative and her period still does not arrive, it would be prudent for her to consult a healthcare professional to explore other potential causes for the missed period, such as hormonal imbalances, stress, or other underlying health issues.

In summary, while the risk of pregnancy exists, especially with unprotected intercourse, the symptoms your girlfriend is experiencing could also be attributed to other factors. A home pregnancy test is a straightforward and effective way to clarify the situation. If there are any further concerns or if she experiences additional symptoms, seeking medical advice is always a good step to ensure her health and well-being.
