Itchy followed by bruising?
Hello Doctor: In the past month, I have experienced unexplained bruising on both my left and right arms, each occurring once.
Initially, the affected area felt itchy, and after lightly scratching it, I noticed some blood streaks.
I could also feel a small hard lump, approximately 0.5 cm in size.
The first occurrence was on my left arm, and after I rubbed it, the bruise became very large.
People around me thought I was a victim of domestic violence.
The bruise lasted for over two weeks, and I used ice packs to help it fade, but I can still slightly feel the hard lump.
This time, it happened on my right arm, and it has been a week.
However, I did not rub it this time and applied ice packs earlier than last time, but the bruise still hasn't faded.
The bruise appears as purple spots, and I can feel a hard lump about 1 cm in size, along with a sensation of swelling.
I am unsure what the issue could be.

Hello: It sounds like it could be a case of subcutaneous bleeding.
Please visit the hematology clinic to check your platelet count and coagulation function.
Additionally, make sure to consume plenty of fresh fruits and vegetables to supplement your vitamin C intake.
Thank you!

Unexplained bruises and itching can be concerning, especially when they appear without any clear cause. Based on your description, there are several potential explanations for the symptoms you are experiencing, including bruising, itching, and the presence of hard lumps.
Firstly, it is important to understand that bruising occurs when small blood vessels under the skin break, allowing blood to leak into the surrounding tissue. This can happen due to various reasons, including trauma, even if it seems minor or unnoticed. In your case, the itching and the appearance of hard lumps could indicate a few different possibilities.
1. Allergic Reactions or Dermatitis: Itching can often be a sign of an allergic reaction or contact dermatitis. If you have come into contact with a new substance, such as a soap, lotion, or even certain fabrics, it could cause localized itching and inflammation, leading to bruising if you scratch the area. The hard lumps you feel could be swollen lymph nodes or localized inflammation due to irritation.
2. Vascular Issues: Conditions affecting the blood vessels, such as vasculitis, can lead to unexplained bruising. Vasculitis is an inflammation of the blood vessels that can cause bleeding under the skin. This condition may also present with other symptoms, such as fever or fatigue.
3. Platelet or Clotting Disorders: If your body has issues with blood clotting or low platelet counts (thrombocytopenia), you may bruise easily. Conditions such as idiopathic thrombocytopenic purpura (ITP) or other hematological disorders could lead to increased bruising and may require blood tests for diagnosis.
4. Infections: Certain infections can cause skin changes, including bruising and lumps. For example, a localized skin infection could lead to inflammation and bruising. If you notice any redness, warmth, or increased pain in the area, it may be worth consulting a healthcare provider.
5. Physical Trauma: Even if you do not recall a specific incident, it is possible that minor trauma could have occurred without your awareness. This could include bumping into objects or engaging in activities that put stress on your arms.
6. Other Medical Conditions: Conditions such as liver disease, vitamin deficiencies (especially Vitamin C or K), or certain medications (like anticoagulants or corticosteroids) can also lead to easy bruising. If you are on any medications, it is important to review them with your healthcare provider.
Given the persistence of your symptoms, especially the hard lumps and prolonged bruising, it is advisable to seek medical attention. A healthcare professional can perform a thorough examination, possibly including blood tests, to determine the underlying cause of your symptoms. They may also assess for any signs of systemic issues that could be contributing to your condition.
In summary, while there are several potential causes for the unexplained bruises and itching you are experiencing, it is crucial to consult with a healthcare provider for an accurate diagnosis and appropriate management. Early intervention can help address any underlying issues and provide peace of mind.
