CA72-4 is abnormal?

Dear Doctor, during my health check-up, my CA72-4 level was 11.1 (should be less than 6.89), but other markers such as CEA, AFP, CA19-9, CA125, CA153, and NSE were normal.
An abdominal ultrasound showed moderate fatty liver.
What further tests should I consider? Thank you.

Elevated levels of tumor markers, such as CA72-4, can be concerning and warrant further evaluation, especially when they exceed the normal range. In your case, a CA72-4 level of 11.1 (with a normal range being less than 6.89) indicates a potential issue that should be investigated further, even though other tumor markers like CEA, AFP, CA19-9, CA125, CA153, and NSE are within normal limits.

CA72-4 is primarily associated with gastric cancer, but it can also be elevated in other conditions, including certain benign diseases and other malignancies. The fact that you have a moderate fatty liver (as indicated by your abdominal ultrasound) could also be a contributing factor to the elevated CA72-4 levels, as liver conditions can sometimes influence tumor marker levels.

Next Steps for Evaluation:
1. Consultation with a Specialist: It is crucial to discuss your elevated CA72-4 levels with a gastroenterologist or an oncologist. They can provide a more tailored approach based on your medical history, symptoms, and the results of your health check-up.

2. Repeat Testing: Sometimes, tumor markers can fluctuate due to various factors, including inflammation or benign conditions. Your doctor may recommend repeating the CA72-4 test after a certain period to see if the levels remain elevated.

3. Imaging Studies: Given the elevated CA72-4 levels and the presence of fatty liver, further imaging studies may be warranted. This could include:
- CT Scan or MRI of the Abdomen: These imaging modalities can provide a more detailed view of the liver and surrounding organs to check for any masses or abnormalities that might not have been visible on ultrasound.

- Endoscopy: If there is a suspicion of gastric cancer or other gastrointestinal issues, an endoscopy may be performed to visualize the stomach and possibly obtain biopsies.

4. Liver Function Tests: Since you have moderate fatty liver, it would be prudent to evaluate your liver function through blood tests. This can help determine if there are any underlying liver issues that need to be addressed.

5. Consideration of Other Conditions: Your healthcare provider may also consider other conditions that could lead to elevated CA72-4 levels, such as pancreatitis or other gastrointestinal disorders. A thorough clinical evaluation is essential.

6. Monitoring: If no immediate cause is found, your doctor may suggest regular monitoring of the CA72-4 levels along with other relevant tests to track any changes over time.

Conclusion:
While an elevated CA72-4 level can be alarming, it is essential to approach the situation with a comprehensive evaluation strategy. Collaborating with your healthcare provider to determine the most appropriate next steps is crucial. They will consider your overall health, symptoms, and any other risk factors you may have. Remember, early detection and intervention are key in managing any potential health issues effectively.
