Scrotal skin itching

Hello Dr.
Hsu,
I have been experiencing itching on the right side of my scrotal skin, especially during the summer.
After a while, I noticed a small bump, about the size of a mung bean, which sometimes itches a lot.
I applied antifungal cream on my own, and it improved somewhat.
However, this summer, the left side also started to itch, and even as we enter winter, I still feel itchy.
I noticed that the areas of itching on my scrotal skin appear white.
I'm wondering if continuing to apply the antifungal cream will help improve this condition.
Additionally, I am preparing for conception, and I am concerned whether using this cream might affect fertility.
Should I be concerned about the bump that is the size of a mung bean? Thank you for your response.

More Info

Scrotal itching can be a common yet distressing issue for many men, and understanding its causes, treatments, and potential implications for fertility is crucial, especially when planning for a family. Based on your description, it seems you are experiencing persistent itching on the scrotum, with the presence of a small bump and changes in skin color. Let's break down the potential causes and treatments for your condition, as well as address your concerns regarding fertility.

Causes of Scrotal Itching
1. Fungal Infections: One of the most common causes of scrotal itching is a fungal infection, such as tinea cruris (jock itch). This condition often presents with red, itchy patches and can spread to the scrotum from the groin area. The fact that you have previously used antifungal medication suggests this could be a possibility.

2. Contact Dermatitis: Irritation from soaps, detergents, or personal care products can lead to contact dermatitis, resulting in itching and discomfort. This is particularly relevant if you have recently changed any products.

3. Skin Conditions: Conditions such as eczema or psoriasis can also affect the scrotal area, leading to itching and changes in skin appearance. The white patches you mentioned could be indicative of a skin condition that requires further evaluation.

4. Infections: Bacterial infections or sexually transmitted infections (STIs) can cause itching and discomfort. If there is any discharge or unusual symptoms, it is essential to seek medical advice.

5. Hygiene Issues: Poor hygiene can lead to irritation and itching. Ensuring proper cleaning and drying of the area can help alleviate symptoms.

Treatments
1. Topical Antifungals: If a fungal infection is suspected, continuing with antifungal creams may be beneficial. However, it is crucial to ensure that the treatment is appropriate for the specific type of infection.

2. Hydrocortisone Cream: For inflammation and itching, a mild over-the-counter hydrocortisone cream can help reduce symptoms. However, it should be used sparingly and not for extended periods.

3. Avoid Irritants: Identifying and avoiding any potential irritants, such as certain soaps or fabrics, can help reduce symptoms.

4. Consult a Dermatologist: Given the persistence of your symptoms and the appearance of the bump, it would be wise to consult a dermatologist or urologist. They can provide a proper diagnosis and recommend specific treatments.

Fertility Concerns
Regarding your concern about fertility, most topical antifungal medications are considered safe and should not affect sperm quality or fertility. However, it is always best to consult with your healthcare provider about any medications you are using, especially when planning for conception.

The small bump you mentioned, which is about the size of a green bean, should also be evaluated by a healthcare professional. While it may be benign, such as a cyst or a sebaceous gland issue, it is essential to rule out any potential concerns, especially if it changes in size, color, or sensation.

Conclusion
In summary, scrotal itching can arise from various causes, including fungal infections, dermatitis, or other skin conditions. While topical treatments may provide relief, it is essential to seek a professional evaluation to ensure an accurate diagnosis and appropriate treatment. Your fertility should not be adversely affected by the use of antifungal medications, but consulting with a healthcare provider is always advisable to address any specific concerns. Taking proactive steps to manage your symptoms and seeking medical advice will help you maintain both your health and your fertility as you prepare for parenthood.
