After holding urine in the penis

Dear Dr.
Hsu, after holding in urine and then relieving myself, my penis appears to be in a state similar to pre-ejaculation for about 5-6 hours.
During this time, it alternates between erect and flaccid, remaining in a semi-erect state.
Even though I have urinated a long time ago, I still feel the sensation of impending ejaculation, and occasionally, prostatic fluid is released.
As a result, I experienced a nocturnal emission that night.
What could this situation be? Thank you.

Mr. Lin, 20~29 year old female. Ask Date: 2011/07/10

Dr. Xu Weikai reply Urology

Mr.
Lin: Bladder distension can indeed lead to an erection.
It may be caused by excessive nerve stimulation.
However, there could be other reasons as well, such as blood disorders or malignancies, that can result in abnormal erections.
Additionally, if the urine in your bladder is not completely emptied, it could also lead to this situation.

The sensations you are experiencing after urination, particularly the feeling of persistent arousal and the release of pre-ejaculatory fluid, can be attributed to several physiological and psychological factors. It is important to understand that the male reproductive and urinary systems are closely linked, and sensations in one can often affect the other.

Firstly, the sensation of a lingering urge to ejaculate or the feeling of being in a state of arousal after urination can be related to the way the body responds to the release of tension. When you hold urine for an extended period, the bladder becomes distended, and the nerves associated with both the bladder and the penis can become overstimulated. This overstimulation can lead to sensations that mimic arousal or even the feeling of needing to ejaculate, even after urination has occurred.

Moreover, the prostate gland, which is responsible for producing a significant portion of seminal fluid, can also be affected. After urination, if the prostate is still somewhat engorged or stimulated, it may lead to the release of pre-ejaculatory fluid. This fluid is often released in response to sexual arousal, but it can also occur due to the physical sensations associated with a full bladder or the act of urination itself.

The phenomenon of nocturnal emissions, or "wet dreams," that you mentioned can also be linked to these sensations. During sleep, particularly during REM (Rapid Eye Movement) sleep, the body can experience spontaneous erections and even ejaculation. If you have been experiencing heightened arousal during the day, it is possible that this could carry over into your sleep, leading to a nocturnal emission.

It is also worth noting that psychological factors can play a significant role in these sensations. Anxiety or stress related to urination, sexual performance, or even the fear of potential medical issues can heighten sensitivity and lead to unusual sensations. If you are preoccupied with thoughts about your urinary health or sexual function, this can create a feedback loop that exacerbates the sensations you are feeling.

In terms of medical considerations, if these sensations are accompanied by pain, discomfort, or changes in urinary habits (such as increased frequency, urgency, or difficulty urinating), it would be advisable to consult a healthcare professional. Conditions such as prostatitis (inflammation of the prostate), urinary tract infections, or other urological issues could potentially be at play.

To summarize, the sensations you are experiencing after urination are likely a combination of physiological responses to bladder distension, prostate stimulation, and psychological factors. If these sensations are causing you distress or if you notice any additional symptoms, it would be prudent to seek medical advice for a thorough evaluation. Maintaining good hydration, practicing relaxation techniques, and ensuring regular urination can help mitigate some of these sensations.
