Glans red spots
Hello doctor, I have recently developed many small red spots around my glans and foreskin that are very itchy! I'm too shy to know what to do.
During the summer vacation, I've been swimming frequently, almost every day.
After swimming, I rinse with plain water without using shampoo or body wash, and I let my swim trunks air dry without rinsing them.
Could this be related to my condition? What should I do now? Can I recover on my own? Thank you.
A-Hang, 20~29 year old female. Ask Date: 2011/08/16
Dr. Xu Weikai reply Urology
Ah Hang: Hello, it may be due to local irritation or a combined infection.
It is strongly advised not to use any soaps or cleaning products (including lotions) for cleaning the glans.
It is recommended to use only cold water for cleaning, and not warm water.
Additionally, after cleaning, applying petroleum jelly to the glans and scrotum may be effective.
If you have any further questions, feel free to ask, and we can discuss and research together.

More Info
Red spots on the glans (the head of the penis) accompanied by itching can be concerning and may indicate several potential issues. Given your recent swimming habits and the lack of proper hygiene after swimming, it is possible that these red spots are related to irritation, infection, or an allergic reaction. Let's explore the possible causes and solutions in detail.
Possible Causes of Red Spots on the Glans
1. Irritation or Allergic Reaction: The materials in swimwear, such as synthetic fabrics, can sometimes cause irritation or an allergic reaction, especially if they are damp for extended periods. Additionally, exposure to chlorine in swimming pools can also lead to skin irritation.
2. Fungal Infections: The warm, moist environment created by swimming and not properly drying off can promote fungal infections, such as candidiasis (yeast infection). This is particularly common in areas that are often covered and can trap moisture.
3. Bacterial Infections: Similar to fungal infections, bacteria can thrive in moist environments. If there is any break in the skin, bacteria can enter and cause an infection, leading to red spots and itching.
4. Sexually Transmitted Infections (STIs): While it may be uncomfortable to consider, some STIs can present with symptoms like red spots or lesions on the genital area. If you have had unprotected sexual contact, this should be considered.
5. Dermatitis: Contact dermatitis can occur due to exposure to irritants or allergens. This could be from soaps, lotions, or even the materials in your swimwear.
Recommended Solutions
1. Maintain Hygiene: It is crucial to maintain proper hygiene, especially after swimming. Rinse off thoroughly with clean water after swimming to remove chlorine and other chemicals. Ensure that the area is dried properly to prevent moisture buildup.
2. Avoid Irritants: If you suspect that your swimwear or any products you use may be causing irritation, consider switching to breathable, cotton-based materials and using hypoallergenic soaps.
3. Topical Treatments: Over-the-counter antifungal creams can be effective if a fungal infection is suspected. Hydrocortisone cream may help reduce inflammation and itching. However, it's essential to consult a healthcare provider before starting any treatment.
4. Avoid Scratching: While it may be tempting, scratching can worsen the irritation and lead to secondary infections. Try to keep the area as dry and clean as possible.
5. Consult a Healthcare Provider: If the red spots persist, worsen, or are accompanied by other symptoms (such as discharge, pain, or fever), it is essential to seek medical attention. A healthcare provider can perform a thorough examination and may conduct tests to determine the exact cause of your symptoms.
6. Consider STI Testing: If there is any possibility of exposure to STIs, it is wise to get tested. Early detection and treatment are crucial for managing STIs effectively.
Conclusion
In summary, the red spots and itching you are experiencing on the glans could be due to various factors, including irritation, infections, or allergic reactions. Proper hygiene, avoiding irritants, and seeking medical advice are essential steps to address the issue. If symptoms persist or worsen, do not hesitate to consult a healthcare professional for a thorough evaluation and appropriate treatment. Taking care of your genital health is important, and addressing these symptoms promptly can help prevent further complications.
