There are red spots on the glans?

Hello Doctor: I would like to ask a question.
I am currently 17 years old and have a habit of masturbation, sometimes once a day and other times every two days.
Now I have small red spots on the glans that do not itch or hurt unless I touch them, which causes pain.
I am unsure what this condition is! I clean my glans almost every day, but the symptoms seem to persist.
Someone suggested that I buy a topical treatment like Betamethasone and stop masturbating for a few days, and that it would resolve the issue.
I am not sure if this is correct, and I am worried that using it might worsen the symptoms, so I wanted to consult with a doctor first.

Dr. Xu Weikai reply Urology

A-Hong: Hello, I recommend cleaning the glans with clean water, preferably cold water, and absolutely avoid using any cleaning products, including soap and body wash.

When it comes to red spots on the glans (the head of the penis), there are several potential causes that could explain your symptoms. Given that you are 17 years old and have a habit of masturbation, it’s important to consider both hygiene and possible dermatological conditions.

1. Irritation or Friction: Frequent masturbation can lead to irritation of the skin on the glans, especially if done without adequate lubrication. This irritation can manifest as small red spots or bumps. If the skin is being rubbed too much or too often, it can become inflamed, leading to discomfort or pain when touched.

2. Infections: While you mentioned that the spots are not itchy or painful unless touched, it’s still possible that a mild infection could be present. Conditions such as balanitis (inflammation of the glans) can occur, often due to poor hygiene or an overgrowth of yeast or bacteria. This condition can lead to redness, swelling, and sometimes discharge.

3. Allergic Reactions: Sometimes, products used for cleaning or lubricating can cause allergic reactions. If you are using soaps, lotions, or other products that may irritate the sensitive skin of the glans, this could lead to red spots. It’s advisable to use mild, unscented soaps and avoid harsh chemicals.

4. Pearly Penile Papules: These are small, harmless bumps that can appear around the glans and are often mistaken for something more serious. They are a normal anatomical variation and do not require treatment.

5. Fordyce Spots: These are small, painless, pale bumps that can appear on the shaft of the penis or the glans. They are sebaceous glands that are not associated with any disease and are completely benign.

6. Sexually Transmitted Infections (STIs): While your symptoms do not strongly suggest an STI, it is still a possibility, especially if you have had unprotected sexual contact. Symptoms of STIs can vary widely, so if there is any concern about potential exposure, it’s important to get tested.

Recommendations:
- Hygiene: Continue to maintain good hygiene by gently cleaning the area with mild soap and water. Avoid using harsh soaps or scrubbing too hard, as this can exacerbate irritation.

- Avoid Irritation: If you suspect that masturbation is causing irritation, consider taking a break for a few days to allow the area to heal. If you choose to continue, using a water-based lubricant can help reduce friction.

- Over-the-Counter Treatments: While some suggest using topical treatments like hydrocortisone or antifungal creams, it’s best to consult a healthcare professional before applying any medication. Self-diagnosing and treating can sometimes worsen the condition.

- Consult a Doctor: If the red spots persist, worsen, or if you develop any additional symptoms (such as itching, pain, or discharge), it’s crucial to see a healthcare provider. They can perform a physical examination and may recommend tests to determine the exact cause of your symptoms.

- Avoid Self-Medication: While it might be tempting to try over-the-counter remedies, it’s important to have a proper diagnosis first. Using the wrong treatment could lead to further irritation or complications.

In summary, while red spots on the glans can be concerning, many causes are benign and easily treatable. Maintaining good hygiene, avoiding irritation, and consulting with a healthcare professional are the best steps to take for your health and peace of mind.
