Epididymitis issues?

Hello, Dr.
Hsu! A month ago, I had sexual intercourse with my girlfriend three times in a short period (all with condoms).
After the last ejaculation, I felt discomfort near the left groin area.
I went to a urology clinic for an examination, and the doctor diagnosed it as epididymitis (urinalysis results were normal) and prescribed antibiotics and painkillers (but since the pain was manageable, I didn't take the painkillers).
After resting and taking the medication for about a week, I had sexual intercourse with my girlfriend again (also with a condom), and similar discomfort occurred after ejaculation, though it wasn't unbearable.
The next day, feeling uneasy, I visited National Taiwan University Hospital.
After listening to my description and examining me, the doctor didn't provide a clear diagnosis but suggested it might be an infection and arranged for urinalysis and medication, planning for an ultrasound during the follow-up visit.

I would like to ask if it is normal to only feel discomfort after ejaculation if it is indeed epididymitis, especially since I usually don't feel anything unusual.
Additionally, the doctor mentioned that I can still masturbate or engage in sexual intercourse (with a condom) because there is still a sexual need, but I am concerned about experiencing pain or discomfort after ejaculation, which makes me hesitant.
Is it really okay to proceed with ejaculation? If I do ejaculate, could it lead to any long-term complications? Thank you very much.

More Info

Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. It can be caused by various factors, including bacterial infections, sexually transmitted infections (STIs), or non-infectious causes such as trauma or certain medical conditions. Understanding the symptoms, treatment options, and concerns regarding sexual activity is crucial for managing this condition effectively.

Symptoms of Epididymitis
The primary symptoms of epididymitis include:
1. Pain and Discomfort: This is often localized to one side of the scrotum and can radiate to the groin or lower abdomen. The pain may worsen with ejaculation or during physical activity.

2. Swelling: The affected epididymis may become swollen and tender.

3. Redness and Warmth: The skin over the affected area may appear red and feel warm to the touch.

4. Urinary Symptoms: These can include a burning sensation during urination, increased urgency or frequency of urination, and sometimes fever or chills.

In your case, experiencing discomfort primarily after ejaculation is not uncommon. This can occur due to the increased pressure and activity in the reproductive system during ejaculation, which may exacerbate any existing inflammation.

Treatment Options
Treatment for epididymitis typically involves:
1. Antibiotics: If the condition is caused by a bacterial infection, antibiotics are the primary treatment. It's essential to complete the full course of antibiotics even if symptoms improve.

2. Pain Relief: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage discomfort.

3. Rest and Ice: Resting and applying ice packs to the affected area can reduce swelling and pain.

4. Supportive Underwear: Wearing supportive underwear can help alleviate discomfort.

Concerns Regarding Sexual Activity
Regarding your concerns about engaging in sexual activity while experiencing symptoms of epididymitis, it is generally advised to proceed with caution:
1. Using Protection: Continuing to use condoms during sexual activity is essential to prevent the spread of any potential infections, especially if the cause of your epididymitis is an STI.

2. Listening to Your Body: If you experience discomfort during or after sexual activity, it may be wise to limit such activities until you have fully recovered. Engaging in sexual activity should not cause significant pain or discomfort.

3. Ejaculation and Pain: It is not uncommon for individuals with epididymitis to experience discomfort after ejaculation. However, if the pain is severe or persistent, it is crucial to consult your healthcare provider for further evaluation.

Potential Long-term Effects
If left untreated, epididymitis can lead to complications such as:
- Chronic Pain: Persistent discomfort in the scrotal area.

- Abscess Formation: Pockets of pus that can develop in the epididymis.

- Infertility: In severe cases, untreated epididymitis can affect sperm production and lead to infertility.

Conclusion
In summary, while experiencing discomfort after ejaculation can be a symptom of epididymitis, it is essential to monitor your symptoms closely and communicate with your healthcare provider. Engaging in sexual activity while using protection is generally acceptable, but you should prioritize your comfort and well-being. If you notice any worsening of symptoms or new concerns, do not hesitate to seek further medical advice. Your health and peace of mind are paramount, and addressing these issues with your doctor can help ensure a full recovery.
