White discharge from the penis during bowel movements (semen?)
Hello Dr.
Hsu, my name is Xiaokai, and I am 20 years old.
I would like to ask you about a situation I have been experiencing.
When I have a bowel movement, I notice that a white substance sometimes comes out of my penis.
This has only happened twice, and I also seem to have some frequent urination issues (I’m not sure if it’s frequent urination, but I feel the urge to urinate again within about half an hour after finishing).
I did some research online and found that this could be related to prostatitis.
If I go to the urology department at the hospital, what kind of examinations and treatments can I expect? How long does treatment usually take, and is it possible to fully recover? Are there any dietary considerations I should be aware of? I have a habit of jogging; can I continue jogging with this condition? Additionally, I have noticed that I have been experiencing fewer erections lately, and when I do achieve an erection, it tends to go soft quickly without continued stimulation.
Also, I have been noticing more clear fluid (prostatic fluid?) coming out after an erection than before.
Is this related to prostatitis? If so, will my condition improve after recovering from prostatitis? I apologize for the many questions, and I really appreciate your time in answering them.
I am very anxious about this...
Thank you again, Dr.
Hsu.

Dr. Xu Weikai reply Urology
Xiao Kai: Hello, prostatitis is not related to jogging.
Additionally, prostatitis requires clinical examination for diagnosis.
It is best to have more information for an accurate diagnosis.

Hello, 小愷. Thank you for reaching out with your concerns. It’s understandable to feel anxious about changes in your body, especially when they involve symptoms that are unfamiliar or concerning. Let’s break down your questions and address them one by one.
Firstly, the white discharge from your penis during bowel movements could be related to a few different factors. One possibility is that it could be pre-ejaculatory fluid, which is a normal secretion from the Cowper's glands that can sometimes be more noticeable during physical exertion, such as straining during a bowel movement. However, if this discharge is accompanied by other symptoms like pain, burning, or changes in urination, it could indicate an underlying issue, such as prostatitis (inflammation of the prostate gland).
Prostatitis can indeed cause symptoms like pelvic pain, urinary frequency, and changes in sexual function, including difficulties with erections. Given that you mentioned experiencing more frequent urges to urinate and changes in your erection quality, it would be wise to consult a urologist. They can perform a thorough evaluation, which may include a physical examination, urine tests, and possibly imaging studies or prostate-specific antigen (PSA) tests, depending on your symptoms and medical history.
In terms of treatment, prostatitis can vary in duration and severity. Acute prostatitis may require antibiotics and can improve within a few days to weeks, while chronic prostatitis may take longer to treat and often involves a combination of medications, lifestyle changes, and sometimes physical therapy. It’s important to follow your doctor’s recommendations closely and attend follow-up appointments to monitor your progress.
Regarding your concerns about diet and lifestyle, maintaining a healthy diet can support your overall health and potentially alleviate some symptoms. Staying hydrated, reducing caffeine and alcohol intake, and avoiding spicy foods can be beneficial. Regular exercise, like your habit of jogging, is generally good for your health, but if you experience pain or discomfort while exercising, it may be wise to modify your routine until you have a clearer understanding of your condition.
As for your concerns about erectile function and the increased amount of pre-ejaculatory fluid, these can also be linked to prostatitis. Many men find that their sexual function improves with appropriate treatment for prostatitis. It’s important to communicate openly with your healthcare provider about these symptoms, as they can provide tailored advice and treatment options.
In summary, while some of your symptoms could be benign, it’s crucial to consult with a urologist to rule out any serious conditions and to receive appropriate treatment. Don’t hesitate to seek help; your health is important, and addressing these concerns early can lead to better outcomes. Remember, you are not alone in this, and many men experience similar issues. Take care, and I hope you find relief soon!
