Touching the glans may cause sharp pain?

I have phimosis, and my glans only becomes exposed during bathing.
When not erect, I can wash with soap and water without any discomfort.
However, I experience a stinging sensation when touching it in a dry state.
When erect, I do not feel stinging from washing with soap, but using plain water causes stinging.
I would like to know if this stinging sensation is due to the glans not being frequently exposed, making it more sensitive.
If I gently touch the glans during prolonged dry conditions or use water on an erect glans, will the stinging sensation improve? Thank you.

Xiao Ming: Hello, to clean the glans, do not use any cleaning agents or hot water.
It is recommended to use petroleum jelly for maintenance after cleaning.

Understanding sensitivity and pain in the glans, particularly in the context of phimosis (a condition where the foreskin cannot be easily retracted), can be complex. Your description indicates that you experience discomfort primarily when the glans is dry and touched, while you do not feel pain when it is cleaned with soap or when it is in a relaxed state. This suggests that the sensitivity may indeed be related to the lack of exposure and moisture of the glans.

The glans penis is typically more sensitive than other areas of the penis due to a higher concentration of nerve endings. In individuals with phimosis, the glans is often covered by the foreskin, which can lead to increased sensitivity when it is finally exposed. This is because the glans is not accustomed to direct contact with air or other surfaces, leading to discomfort or pain when it is touched, especially in a dry state.

When you mention that you do not experience pain when using soap, it is likely because the soap creates a lubricating effect that reduces friction. However, when using plain water, the lack of lubrication can lead to a more pronounced sensation of discomfort, especially if the glans is dry.
To address your concerns, here are some potential solutions and considerations:
1. Moisturization: Regularly applying a gentle, fragrance-free moisturizer or a specialized lubricant can help keep the glans hydrated. This may reduce sensitivity and discomfort when it is touched, particularly in dry conditions.

2. Gradual Exposure: If comfortable, gradually exposing the glans to air and gentle touch can help desensitize the area over time. This should be done carefully to avoid any pain or discomfort.

3. Hygiene Practices: Maintaining good hygiene is essential. While you mentioned that you clean the area during showers, ensure that you are doing so gently and not causing irritation. Avoid harsh soaps that can dry out the skin.

4. Consultation with a Specialist: If the sensitivity persists or worsens, it may be beneficial to consult a urologist. They can assess whether the phimosis is contributing to your symptoms and discuss potential treatments, which may include options like circumcision or other procedures to alleviate the condition.

5. Avoiding Irritants: Be cautious with any products that may irritate the sensitive skin of the glans. This includes scented soaps, lotions, or other personal care products that may cause allergic reactions or irritation.

6. Monitoring Symptoms: Keep track of when the sensitivity occurs and any other accompanying symptoms. If you notice changes, such as increased pain, swelling, or discharge, seek medical attention promptly.

In summary, your sensitivity and pain in the glans may be related to the lack of exposure and moisture, leading to increased sensitivity. Implementing proper hygiene, moisturization, and gradual exposure may help alleviate discomfort. However, if symptoms persist, consulting a healthcare professional is advisable to rule out any underlying conditions and discuss appropriate treatment options.
