Knee healing issues after a car accident?

Hello, doctor.
I would like to ask about an incident that occurred at the end of last month.
I was in a car accident and only sustained abrasions on both knees.
However, after the accident and before making contact with the ground, it seemed that there was an abnormal range of motion in my knees.
Now that they are healing, my left knee cannot support much weight.
Three hours ago, I accidentally lifted my foot to step onto the bed and caught my foot, which caused sudden severe pain in the back of my knee (the popliteal area) and I was unable to apply any force for about five minutes.
Doctor, is it possible that I have injured a tendon?

Da Tou, 20~29 year old female. Ask Date: 2013/01/05

Dr. Shi Guozheng reply Orthopedics

When you refer to injuring a tendon, are you talking about a tendon or ligament? Typically, an injury to a tendon can lead to impaired joint movement or muscle weakness.
On the other hand, an injury to a ligament can cause joint pain or instability.
Based on your symptom description, it seems that neither of these applies to you.
Rest assured, it may just be symptoms following a bruise or contusion, which usually takes about three to six weeks to heal.
Be patient as it recovers; not returning to full strength is just part of the process.

After a car accident, it is not uncommon for individuals to experience lingering pain and discomfort, even if initial X-rays do not show any fractures. Your description of deep pain in the back of your knee (the popliteal area) and difficulty bearing weight suggests that there may be underlying injuries that were not immediately apparent.
Injuries to the knee can involve not just the bones, but also the ligaments, tendons, and cartilage. Ligament injuries, particularly to the anterior cruciate ligament (ACL) or posterior cruciate ligament (PCL), can occur even in the absence of visible fractures. These ligaments are crucial for stabilizing the knee joint, and damage to them can lead to pain, swelling, and instability.
Given that you experienced a sudden, severe pain in the back of your knee after a minor incident, it is possible that you may have strained or injured a ligament or tendon. The popliteal area contains important structures, including the popliteal tendon and the hamstring tendons, which can be affected by trauma. Additionally, the meniscus, which is the cartilage that cushions the knee joint, could also be injured, leading to pain and limited range of motion.

The healing process for soft tissue injuries, such as ligament or tendon injuries, can take several weeks to months, depending on the severity of the injury. While bone injuries may heal within a few weeks, soft tissue injuries often require a longer recovery time. Symptoms such as swelling, pain during movement, and difficulty bearing weight can persist during this period.

It is crucial to seek a thorough evaluation from a healthcare professional, such as an orthopedic specialist or a physical therapist, who can perform a comprehensive assessment of your knee. They may recommend imaging studies, such as an MRI, to visualize the soft tissues and determine if there are any ligamentous injuries or other issues that need to be addressed.
In the meantime, you can manage your symptoms by following the R.I.C.E. protocol: Rest, Ice, Compression, and Elevation. Avoid activities that exacerbate your pain, and consider using over-the-counter pain relievers to help manage discomfort. Physical therapy may also be beneficial in restoring strength and range of motion to your knee.

In summary, while your initial X-rays did not show fractures, it is entirely possible that you have sustained a ligament or soft tissue injury that requires further evaluation and treatment. Prompt medical attention will help ensure that you receive the appropriate care to facilitate your recovery and prevent any long-term complications.
