Foreskin issues
Hello, Dr.
Hsu.
I have a question.
When I was in elementary school, I underwent a circumcision, and now during an erection, the entire glans is exposed.
The foreskin is completely retracted.
I'm not sure if this is the reason, but when I first penetrate my girlfriend, it's fine, but as we start to move, she feels pain.
This is quite troubling for me.
Could it be that I had too much foreskin removed, causing her discomfort? If so, is there a way to address this issue?

Dr. Xu Weikai reply Urology
Ahao: Hello, whether or not the foreskin has been circumcised does not have a significant relationship with the pain experienced by the female partner.
Generally, pain is often caused by insufficient lubrication.
If the female partner does not secrete enough lubrication, it can lead to dryness, which naturally causes discomfort.
I recommend enhancing foreplay to ensure that the female partner's vagina is adequately lubricated and relaxed before intercourse.
This can help reduce pain during sexual activity.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Responses on the internet should only be taken as references; if necessary, it is important to seek medical attention at a healthcare facility.

Understanding pain during intercourse can be a complex issue, and it is important to consider various factors that could contribute to this discomfort. In your case, you mentioned having undergone circumcision as a child, and you are experiencing pain during intercourse, which your girlfriend also feels. Let's explore the potential connections between circumcision and pain during sexual activity, as well as other possible causes.
Circumcision involves the surgical removal of the foreskin, which can lead to changes in sensitivity and the way the glans (the head of the penis) interacts with friction during intercourse. Some men report increased sensitivity after circumcision, while others may experience decreased sensitivity. If the circumcision was performed in a way that left the glans overly exposed or if there was excessive removal of tissue, it could potentially lead to discomfort during sexual activity for both partners.
For your girlfriend, pain during intercourse could be attributed to several factors, including:
1. Vaginal Dryness: Insufficient lubrication can lead to friction and discomfort during intercourse. This can be due to hormonal changes, stress, or lack of arousal. Using a water-based lubricant can help alleviate this issue.
2. Vaginismus: This is a condition where involuntary muscle contractions occur in the pelvic floor muscles, making penetration painful or difficult. It can be caused by anxiety, past trauma, or other psychological factors.
3. Infections or Inflammation: Conditions such as yeast infections, bacterial vaginosis, or sexually transmitted infections can cause pain during intercourse. It is important for your girlfriend to consult a healthcare provider if she experiences persistent pain.
4. Anatomical Issues: Sometimes, anatomical variations or conditions such as fibroids or endometriosis can lead to discomfort during intercourse.
Given your concerns about circumcision, it is worth noting that while circumcision itself is not typically associated with pain during intercourse, the way it was performed could play a role in your experience. If you feel that the circumcision may have been excessive or if you have any concerns about the appearance or function of your penis, it would be advisable to consult a urologist. They can assess whether there are any anatomical issues that could be contributing to the pain.
Additionally, communication with your partner is crucial. Discussing what feels comfortable and what does not can help both of you navigate this sensitive issue. If the pain persists despite trying lubricants and ensuring adequate arousal, it may be beneficial for both of you to seek guidance from a healthcare professional. A sexual health specialist or a therapist who specializes in sexual health can provide valuable insights and strategies to improve your sexual experiences.
In summary, while circumcision could potentially influence sensitivity and comfort during intercourse, it is essential to consider a range of factors that could be contributing to the pain experienced by both you and your girlfriend. Consulting with healthcare professionals can help identify the underlying causes and provide appropriate solutions to enhance your sexual health and relationship.
