There are many small white spots on the penis?
Hello doctor, I have a troubling question.
Yesterday while I was showering, I noticed many small white spots on the top of my penis.
They are not painful or itchy, but they look quite unsightly, especially on the back of the penis, and it feels really disgusting.
Could this be related to excessive masturbation and not cleaning the foreskin properly? Besides surgery, is there any way to make these unpleasant spots disappear? Thank you!

The presence of white bumps on the penis can be concerning, but it's important to understand that there are several potential causes, many of which are benign. Here are some common reasons for white bumps on the penis:
1. Pearly Penile Papules: These are small, harmless bumps that typically appear around the corona of the glans (the head of the penis). They are a normal anatomical variation and are not a sign of disease. They do not require treatment unless the individual desires cosmetic removal.
2. Fordyce Spots: These are small, painless, pale bumps that are sebaceous (oil) glands without hair follicles. They are common and completely harmless. Like pearly penile papules, they do not require treatment.
3. Folliculitis: This condition occurs when hair follicles become inflamed, often due to infection or irritation. It can result in small red or white bumps that may be itchy or painful. Good hygiene and avoiding tight clothing can help manage this condition.
4. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They may be raised or flat and can sometimes be itchy. If you suspect genital warts, it’s important to see a healthcare provider for diagnosis and treatment options.
5. Molluscum Contagiosum: This is a viral infection that causes small, firm, raised bumps with a dimple in the center. They can appear anywhere on the body, including the genital area. Treatment is often not necessary, but a healthcare provider can offer options for removal if desired.
6. Lichen Sclerosus: This is a chronic skin condition that can cause white patches or bumps on the genital area. It may require treatment with topical steroids or other medications.
7. Candidiasis (Yeast Infection): An overgrowth of yeast can lead to white patches or bumps, often accompanied by itching or irritation. Antifungal treatments can help resolve this condition.
Given your description of the bumps being painless and not itchy, it is possible that they could be pearly penile papules or Fordyce spots, both of which are benign. However, if you have concerns about hygiene or if you believe that your previous habits (like infrequent washing) may have contributed to this issue, it’s a good idea to improve your hygiene practices. Regular washing with mild soap and water can help maintain genital health.
If the appearance of these bumps is distressing or if you notice any changes (such as pain, itching, or bleeding), it is advisable to consult a healthcare provider or a dermatologist. They can perform a physical examination and may conduct tests if necessary to rule out any sexually transmitted infections or other conditions.
In terms of treatment, many of the benign conditions do not require any intervention. If you are looking for cosmetic removal of pearly penile papules or Fordyce spots, procedures such as laser therapy or cryotherapy can be performed by a qualified healthcare professional. However, it’s essential to weigh the risks and benefits of any procedure.
In conclusion, while the presence of white bumps on the penis can be alarming, many causes are benign and do not require treatment. Maintaining good hygiene and consulting with a healthcare provider for an accurate diagnosis and appropriate management is the best course of action.
