Cracking at the tip of the glans under the foreskin?
Hello: I have a condition of phimosis, where my foreskin covers the penis both when erect and flaccid, and I can retract the foreskin.
However, when I retract it during an erection, I experience swelling.
Over the past year, I have frequently encountered symptoms of cracking of the foreskin, which has made normal sexual intercourse difficult.
Initially, I experienced redness, swelling, and itching of the foreskin (sometimes accompanied by white discharge).
Later, the tip of the foreskin began to crack, and during sexual intercourse, the glans swells, causing pain when I attempt to retract the foreskin.
After sexual activity, my partner also experiences redness, swelling, itching, and excessive discharge in their genital area.
I have consulted several hospitals regarding this issue.
Some doctors suggested that it is a yeast or fungal infection affecting the genital area, while another indicated that it is an infection caused by phimosis.
I have been prescribed medications to apply regularly, but none have led to a cure, and the symptoms keep recurring, which is very distressing! This condition continues to cause intermittent redness of the glans and foreskin; although sometimes it seems to self-heal, it can also become itchier at times.
I am truly unsure how to resolve this situation permanently! I would like to consult a professional physician regarding the following questions: 1.
Is a circumcision necessary? 2.
If surgery is performed, will it be covered by health insurance? 3.
Are there any recommended hospitals or doctors in the northern region (including Taipei, Taoyuan, Hsinchu, and Miaoli) who can provide treatment?

More Info
Recurrent issues with foreskin cracking can be quite distressing and can significantly impact one’s quality of life, particularly when it interferes with sexual activity. Based on your description, it seems that you are experiencing a combination of symptoms that may be related to both a physical condition (such as phimosis) and a possible infection (like a yeast or fungal infection).
Understanding the Symptoms
1. Phimosis: This condition occurs when the foreskin cannot be easily retracted over the glans (the head of the penis). In your case, it appears that you can retract the foreskin, but you experience discomfort and swelling when doing so, especially during an erection. This could indicate that the foreskin is too tight or that there is some inflammation present.
2. Infection: The symptoms of redness, swelling, itching, and the presence of white discharge suggest that there may be an underlying infection. Yeast infections (often caused by Candida) are common in individuals with phimosis, as the warm, moist environment can promote fungal growth. Bacterial infections can also occur, particularly if there is any trauma to the skin from cracking.
3. Cracking and Pain: The cracking of the foreskin, particularly during sexual activity, can be painful and may lead to further complications, including scarring or recurrent infections.
Treatment Options
1. Topical Treatments: It is essential to follow the treatment regimen prescribed by your healthcare provider. Antifungal creams or ointments may be effective if a yeast infection is present. If a bacterial infection is suspected, antibiotics may be necessary. However, if these treatments have not resolved the issue, further evaluation is warranted.
2. Hygiene Practices: Maintaining good hygiene is crucial. Gently cleaning the area with mild soap and water, and ensuring it is thoroughly dried, can help prevent infections. Avoiding irritants, such as harsh soaps or lotions, is also advisable.
3. Consideration of Circumcision: If phimosis is contributing to your recurrent issues, circumcision may be a viable option. This surgical procedure removes the foreskin, which can alleviate the problems associated with tight foreskin and recurrent infections.
- Insurance Coverage: In many cases, health insurance may cover circumcision if it is deemed medically necessary. It’s best to consult with your insurance provider and discuss this with your healthcare provider to understand the specifics of your coverage.
4. Consultation with a Specialist: Given the complexity of your symptoms and the fact that you have seen multiple doctors without resolution, it may be beneficial to consult a urologist. A urologist specializes in male reproductive health and can provide a more comprehensive evaluation and treatment plan.
Recommendations for Finding a Specialist
In the northern regions of Taiwan (including Taipei, Taoyuan, Hsinchu, and Miaoli), there are several reputable hospitals and clinics where you can seek specialized care. Look for hospitals with a urology department that has good reviews for treating conditions related to male reproductive health.
Conclusion
Your situation is understandably frustrating, especially with the recurrent nature of your symptoms. It is essential to continue seeking medical advice and possibly consider a referral to a urologist who can provide a thorough examination and discuss the potential benefits of circumcision if appropriate. Remember, addressing both the physical and potential infectious components of your condition is crucial for achieving long-term relief.
