Penile curvature correction with tunica albuginea folding technique using absorbable sutures?

Hello Dr.
Hsu, I had a penile curvature plication surgery performed by Dr.
Hsieh Ru-Tun at National Taiwan University Hospital on November 11th.
It is now the end of December, and I am still experiencing some subtle pain in my penis.
Could it be that the sutures have not absorbed yet? The appearance of my penis looks normal, with no signs of inflammation or pus.
I am unsure of your familiarity with this type of surgery, but I feel discomfort while walking and cycling, and wearing boxers is more painful compared to wearing briefs.
Is it possible that the sutures are not absorbing, or could it be that the internal wound has not fully healed? It has been 46 days post-surgery.
How long does it typically take for recovery from this type of surgery? Thank you.

Mr. Li, 20~29 year old female. Ask Date: 2014/12/27

Mr.
Li: Hello, some absorbable sutures may take around 180 days to fully absorb.
Local heat application can help increase blood circulation, which may be beneficial.
If you have any further questions, feel free to ask, and we can discuss and research together.

After undergoing penile curvature surgery, it is not uncommon to experience lingering pain or discomfort for several weeks or even months post-operatively. The procedure you had, known as the tunica albuginea plication or folding technique, is designed to correct curvature by altering the tissue structure of the penis. While the surgery is generally considered safe and effective, the recovery process can vary significantly from person to person.

In your case, experiencing dull pain 46 days post-surgery can be attributed to several factors. Firstly, the healing process of soft tissues can take time, and it is normal for some discomfort to persist as the body repairs itself. The use of absorbable sutures means that they will gradually dissolve over time, but the healing of the surrounding tissues may still be ongoing. If the sutures are still present, they may cause some irritation, but this should diminish as they dissolve.

The pain you describe while walking, riding a bike, or wearing certain types of underwear could be related to several factors:
1. Healing Process: The surgical site may still be sensitive as the tissues heal. This sensitivity can lead to discomfort during physical activities or when pressure is applied to the area.

2. Nerve Sensitivity: Post-surgical pain can sometimes be due to nerve irritation or sensitivity in the area. This can manifest as a dull ache or sharp pain, especially during movement.

3. Scar Tissue Formation: As the incision heals, scar tissue can form, which may be less flexible than the surrounding tissue. This can lead to discomfort, especially if the scar tissue is tight or adherent to other structures.

4. Activity Level: Engaging in activities that put strain on the surgical site, such as cycling or wearing tight underwear, can exacerbate discomfort. It may be beneficial to wear looser-fitting clothing and avoid activities that cause pain until you feel more comfortable.

Regarding your concern about the sutures not absorbing properly, it is essential to follow up with your surgeon if you notice any unusual symptoms, such as increased pain, swelling, or signs of infection. However, if the external appearance of the surgical site is normal and there are no signs of inflammation or infection, it is likely that the healing process is proceeding as expected.

In terms of recovery time, while many patients begin to feel significant improvement within a few weeks, complete healing can take several months. It is essential to be patient and allow your body the time it needs to heal fully. If the pain persists or worsens, or if you have any concerns about your recovery, it is advisable to consult your surgeon for a thorough evaluation. They may recommend physical therapy or other interventions to help manage pain and improve recovery.

In summary, while some discomfort after penile curvature surgery is normal, it is crucial to monitor your symptoms and communicate with your healthcare provider about any concerns. They can provide personalized advice based on your specific situation and help ensure a smooth recovery process.
