After circumcision

Hello, Director Hsu.
I had a circumcision on March 6, and it has been over a month since then.
The wound has healed, and everything seems fine, but now I feel some tightness at the circular suture site during erections, which is a bit uncomfortable.
Is this normal? How can I improve this condition? Can I apply any topical medications like steroids? Thank you for your guidance.

W, 20~29 year old female. Ask Date: 2015/04/22

Dr. Xu Weikai reply Urology

W: Hello, currently it is not recommended to administer steroids, as they can affect wound healing.
Note: Generally, wound healing takes at least 6-8 weeks to achieve 80-85% of its original strength.
After surgery, there may be swelling, which can cause a feeling of tightness.
Gentle local massage and warm compresses may help improve this.
Note: If tightness persists beyond 3 months, it may be due to scar tissue contracture causing narrowing, which is rare (less than 1%, mostly due to individual predisposition).
However, if it occurs, a minor surgical procedure may be needed for correction.
Alternatively, at that time, a topical steroid ointment can be applied to the tight tissue, which may be effective for some patients.
I hope this information is helpful.

Post-circumcision concerns such as tightness and discomfort are not uncommon, especially in the weeks and months following the procedure. It is important to understand that circumcision involves the removal of the foreskin, which can lead to changes in the sensitivity and elasticity of the skin around the glans (the head of the penis).
In your case, experiencing tightness at the site of the circumcision, particularly along the circumferential suture line, can be attributed to several factors. First, the healing process can sometimes lead to scar tissue formation, which may feel tighter than the surrounding skin. This is especially true if the sutures used were absorbable and are still in the process of being reabsorbed by the body.
Additionally, the sensation of tightness can also be influenced by the degree of swelling or inflammation that may still be present, even if the wound appears to have healed. It is also possible that the skin around the circumcision site has not fully adjusted to the new anatomy, which can lead to discomfort during erections.

To address your concerns, here are a few recommendations:
1. Gentle Stretching: Engaging in gentle stretching exercises of the penile skin can help improve elasticity and reduce tightness. This should be done carefully to avoid causing pain or injury.

2. Moisturizing Creams: Applying a gentle, fragrance-free moisturizer can help keep the skin supple. Look for products that are designed for sensitive skin.

3. Topical Steroids: In some cases, a mild topical steroid cream may be recommended to reduce inflammation and promote healing. However, this should only be done under the guidance of a healthcare professional, as inappropriate use can lead to skin thinning or other complications.

4. Avoiding Irritants: Ensure that you are not using any harsh soaps or products that may irritate the area. Stick to mild, hypoallergenic products.

5. Consultation with a Specialist: If the tightness and discomfort persist, it is advisable to consult with your urologist or the surgeon who performed the circumcision. They can assess the area for any complications such as excessive scar tissue or other issues that may require intervention.

6. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, can help manage discomfort if needed.

It is important to monitor your symptoms closely. If you notice any signs of infection (such as increased redness, swelling, discharge, or fever), or if the discomfort worsens, seek medical attention promptly.
In summary, while some tightness and discomfort post-circumcision can be normal, it is essential to take steps to manage these symptoms and consult with a healthcare provider if they persist or worsen. Your comfort and health are paramount, and a professional evaluation can provide peace of mind and appropriate treatment options.
