Anejaculation
Hello Doctor, last week I inquired about the issue of anejaculation.
I went to the urology department at China Medical University last week, and the doctor advised me to get a blood test.
Today, I reviewed the results, and the doctor informed me that all indicators are normal, including GOT, GPT, male hormones, triglycerides, etc.
The doctor also performed a physical examination, including a prostate exam, and stated that everything is normal.
Finally, the doctor prescribed Bupropion XL 150 mg/tablet & Wellbutrin XL 150 mg/tablet for a 14-day supply (to be taken once before bedtime).
However, I researched this medication online and found that it is used for smoking cessation and antidepressant treatment.
I do not smoke and do not have depression...
So, why did the doctor prescribe this medication to me?

Dr. Fu Shizhang reply Urology
Generally, such medications are used in urology treatment by leveraging their side effects.
If you have any questions regarding the treatment, it is advisable to discuss them with the prescribing physician, as I believe they would be more than happy to provide you with answers.

Anejaculation, or the inability to ejaculate, can be a complex condition with various underlying causes, including psychological factors, neurological issues, hormonal imbalances, and medication side effects. In your case, since the urologist has ruled out many physical causes through blood tests and examinations, it is possible that your condition may have a psychological component or may be related to other factors that are not immediately apparent.
The prescription of Bupropion (also known by its brand names Wellbutrin and Zyban) may seem unusual at first glance, especially since you mentioned that you do not smoke and do not have a diagnosed depressive disorder. However, Bupropion is a unique medication that is primarily used as an antidepressant and smoking cessation aid, but it also has other applications that may be relevant to your situation.
One of the interesting aspects of Bupropion is that it affects the neurotransmitters in the brain, particularly dopamine and norepinephrine. Dopamine is closely linked to the brain's reward system and is involved in many functions, including mood regulation and sexual function. In some cases, medications that influence dopamine levels can help improve sexual function, including ejaculation. Therefore, your doctor may have prescribed Bupropion with the hope that it could help address the anejaculation by enhancing dopamine activity, which might improve sexual arousal and function.
Additionally, Bupropion has been noted to have a lower incidence of sexual side effects compared to other antidepressants, which often lead to issues like delayed ejaculation or anejaculation. This characteristic makes it a potential option for individuals experiencing sexual dysfunction related to other medications or psychological factors.
It is also important to consider that the treatment of sexual dysfunction often requires a multifaceted approach. While medication can play a significant role, addressing psychological factors through therapy, lifestyle changes, and open communication with partners can also be crucial. If anxiety or stress is contributing to your anejaculation, Bupropion may help alleviate some of those symptoms, thereby indirectly improving your sexual function.
If you have concerns about taking Bupropion, especially since you do not have a history of depression or smoking, it is essential to have an open dialogue with your doctor. They can provide further insights into why they chose this medication for you and discuss any potential side effects or alternative treatments that may be available.
In summary, while Bupropion is primarily known for its role in treating depression and aiding smoking cessation, its effects on dopamine and norepinephrine may offer benefits for individuals experiencing anejaculation. It is crucial to work closely with your healthcare provider to monitor your response to the medication and explore additional therapeutic options if necessary.
