After the ligation of internal hemorrhoids..?

Hello, doctor! I have undergone two vasectomies.
The first time was painless, and the next day it fell off.
The second time, however, was painful due to a hemorrhoid that was bleeding, so I had another procedure.
But the pain was significant this time...
I kept feeling the urge to have a bowel movement, and pain relievers and sitz baths didn't help.
Originally, the doctor said it would take about a week for it to fall off, but after three bowel movements the next day, it fell off.
After it fell off, the pain subsided, and there was no more bleeding (the stools were soft and it took less than three minutes to finish).
I don't understand why there was such a difference between the two experiences...
The first procedure was on the left side, and the second was on the right side.
Now, I often feel discomfort around the anus; when I contract, it feels like there's a piece missing on the left side.
When riding a motorcycle, it feels like the sphincter is pressing against the seat, which is uncomfortable.
However, the left side's wound has healed, and I just had the right side done.
Why is the left side particularly uncomfortable...? The doctor scheduled a follow-up appointment in half a month.
During the first procedure, I used suppositories for almost a month; can I still use them now? Although it's not painful, as mentioned, it is uncomfortable, and I'm worried that using them for too long might have side effects.
The sitz baths are somewhat relieving; is there a limit to how many times I can take them? Thank you!

The discomfort associated with hemorrhoid ligation largely depends on the depth of the ligation.
The second ligation may be shallower, which could lead to a sensation of discomfort.
If there is currently no pain or bleeding, and only an unusual feeling is present, there is generally no need to use suppositories.
Sitz baths can be taken as often as desired, provided they do not cause skin irritation due to excessive frequency.

Understanding discomfort after hemorrhoid ligation can be a complex issue, as individual experiences can vary significantly based on several factors, including the specific technique used, the location of the hemorrhoids, and the patient’s overall health.
In your case, it seems that the first ligation was relatively uneventful, with minimal pain and a quick recovery. However, the second ligation was accompanied by significant discomfort, which is not uncommon. Hemorrhoid ligation, particularly for internal hemorrhoids, can lead to varying levels of pain and discomfort depending on the size and location of the hemorrhoid, as well as the individual’s pain threshold and healing response.

The fact that you experienced severe pain after the second ligation could be attributed to several factors. First, if the internal hemorrhoid that was ligated was larger or more inflamed than the first, it could lead to more significant post-procedural discomfort. Additionally, the sensation of needing to have a bowel movement frequently after the procedure can be a common side effect, as the body adjusts to the changes in the area. The ligation process can cause temporary swelling and irritation, which may trigger the urge to defecate.

The rapid resolution of pain and the absence of bleeding after the hemorrhoid fell off is a positive sign, indicating that the ligation was successful. It’s also worth noting that the pain relief you experienced after the hemorrhoid fell off suggests that the ligation was effective in addressing the source of discomfort.

Regarding your ongoing discomfort, it’s not unusual for patients to feel a sensation of fullness or discomfort in the anal area after ligation. This can be due to residual swelling or the healing process. The feeling of “missing a piece” on the left side may be related to the healing tissue or the way the anal sphincter is functioning post-procedure. It’s essential to communicate these sensations to your physician during your follow-up appointment.

As for the use of topical treatments such as suppositories, it’s generally safe to continue using them as directed by your physician, especially if they provide relief. However, it’s crucial to follow your doctor’s advice regarding the duration of use to avoid potential side effects, such as irritation or dependency on the medication for bowel movements.

Sitz baths can be very beneficial for comfort and healing after hemorrhoid procedures. They help to soothe the area, reduce swelling, and promote blood flow, which can aid in healing. While there is no strict limit on the number of sitz baths you can take, moderation is key. Overdoing it may lead to skin irritation or other issues, so it’s best to follow your comfort level and your doctor’s recommendations.

In summary, while your experiences with the two ligations differ, they can be attributed to various factors, including the size and condition of the hemorrhoids, individual pain tolerance, and the body’s healing response. Continue to monitor your symptoms, maintain communication with your healthcare provider, and follow their recommendations for treatment and follow-up care. If your discomfort persists or worsens, don’t hesitate to seek further evaluation. Your health and comfort are paramount, and addressing any concerns promptly can help ensure a smooth recovery.
