There are a lot of small bumps on the foreskin?

Hello, doctor.
I have developed clusters of tiny blisters inside my foreskin, much smaller than sesame seeds.
They are not painful or itchy, and I noticed them about three weeks ago.
I had previously visited sex workers, but I always used a condom during intercourse.
However, I did have oral sex without a condom.
I consulted three doctors who prescribed medication and topical treatments for the foreskin, but it seems to have had no effect.
A few days ago, I had a blood test, and the results showed that I am negative for HIV and syphilis.
What could this condition be? If I had the blood test done one week after seeing a sex worker and the results came back negative, would that be accurate? I hope you can answer my questions.
Thank you.

Hao, 20~29 year old female. Ask Date: 2015/09/02

Based on the literal description, it is not possible to confirm the condition without actually seeing the lesion.
It is recommended to have a physician evaluate it for accurate diagnosis.

Understanding small bumps under the foreskin can be concerning, especially when they appear suddenly and without accompanying symptoms like pain or itching. Based on your description, it sounds like you may be experiencing a benign condition rather than a sexually transmitted infection (STI), especially since your blood tests for HIV and syphilis came back negative.

The small bumps you are describing could be several things. One common cause of small, painless bumps in the genital area is pearly penile papules, which are normal anatomical variations and not a disease. These are small, dome-shaped bumps that typically appear around the corona of the glans (the head of the penis) and are completely harmless. They are often mistaken for warts or other lesions but are a normal part of anatomy.

Another possibility is that these bumps could be sebaceous cysts or folliculitis, which can occur due to blocked hair follicles or sebaceous glands. These conditions are also generally harmless and do not require treatment unless they become infected or bothersome.

In your case, since you mentioned that you have seen multiple doctors and have been treated without improvement, it might be worthwhile to consider that these bumps are not indicative of an infection or disease. If the bumps are not causing any discomfort or changes in your sexual health, they may not require any intervention.

Regarding your concern about the timing of your blood tests, it is important to note that while some STIs can be detected shortly after exposure, others may take longer to show up in tests. For example, HIV can take up to three months to be detectable in some cases, while syphilis can often be detected within a few weeks. If you had a potential exposure and are concerned, it is advisable to follow up with your healthcare provider for further testing after the appropriate window period.

If the bumps persist or change in appearance, or if you develop any new symptoms, it would be prudent to seek further evaluation from a dermatologist or a urologist who specializes in genital health. They can provide a more accurate diagnosis and reassurance.

In summary, while it is understandable to be concerned about new symptoms, the small, painless bumps you are experiencing are likely benign. Continue to monitor the situation, and do not hesitate to seek further medical advice if you have any additional concerns or if the condition changes. Remember, maintaining open communication with your healthcare provider is key to addressing any health concerns you may have.
