Recent issues related to erectile function?

Hello, doctor.
I am currently a third-year junior high school student.
In the past, when I woke up about five minutes after getting up, my penis was still erect, but in the last week or two, I have noticed that the phenomenon of "morning wood" is less pronounced; it feels like I am only half erect.
I also find it harder to achieve an erection lately (I used to get erect easily with sexual fantasies, but now it’s more difficult), and the erection fades quickly.
When it starts to go soft, the sensation in my brain feels the same as when it is fully soft.
However, during masturbation yesterday and today, the duration was similar to before, but after ejaculation, I experienced a strange sensation that felt somewhat painful but not exactly painful (I don’t quite remember if I had this feeling before), and it wasn’t as pleasurable as usual.

Could these symptoms be related to my daily routine? I usually go to bed quite late, often around 1 AM (I’ve been doing this for almost a year), and during the summer vacation, I sometimes study until after 2 AM.
Could it also be related to the increased academic pressure as I prepare for high school? Or is it because I have been masturbating more often during the summer? (At the end of July and beginning of August, I masturbated three times in one week.) However, the symptoms started in September, and the last time I masturbated before the symptoms began was around late August; I only masturbated once after early August.
Are these factors related to my symptoms? If so, how can I improve this situation? Should I seek medical attention or observe for a while longer? Thank you.

Dr. Xu Weikai reply Urology

Lin: Hello, morning erections are not very significant.
They usually indicate that the bladder is full and it's time to get up to urinate.
Note: Each person's erection hardness and penis length can vary.

Understanding the changes in morning erections and overall sexual health during adolescence is crucial, especially for a 15- to 17-year-old navigating the complexities of puberty. Your concerns about the recent changes in your morning erections, sexual arousal, and the sensations experienced during masturbation are valid and deserve a thoughtful response.

Firstly, it's important to recognize that fluctuations in sexual function, including morning erections (also known as nocturnal penile tumescence), are entirely normal during puberty. The frequency and intensity of morning erections can vary significantly from one individual to another and can be influenced by several factors, including hormonal changes, sleep patterns, and psychological stressors.

1. Morning Erections: Typically, males experience several erections during the night, particularly during REM (rapid eye movement) sleep. These occurrences are a natural part of the sleep cycle and can be influenced by various factors such as stress, sleep quality, and overall health. If you've noticed a decrease in the frequency or intensity of your morning erections, it could be related to your sleep habits. Consistently going to bed late, as you mentioned (around 1 AM), can disrupt your sleep cycle and reduce the amount of REM sleep you get, potentially leading to fewer morning erections.

2. Psychological Factors: The pressures of school, especially as you approach the end of middle school and transition into high school, can contribute to increased stress and anxiety. These psychological factors can affect sexual arousal and performance. If you are feeling overwhelmed by academic responsibilities or social pressures, it may be impacting your sexual health. Stress can lead to increased levels of cortisol, which may inhibit testosterone production and affect libido.

3. Masturbation Frequency: While masturbation is a normal part of sexual development, excessive or compulsive masturbation can lead to temporary changes in sexual function. However, the frequency of masturbation alone is unlikely to be the sole cause of the symptoms you're experiencing, especially if the changes began after a period of reduced activity. It's essential to maintain a healthy balance and ensure that it does not interfere with your daily life or responsibilities.

4. Physical Sensations: The unusual sensations you described during masturbation, such as discomfort or a lack of pleasure, could be due to several factors, including anxiety, changes in arousal levels, or even physical issues like tension in the pelvic area. If these sensations persist, it may be worth consulting a healthcare professional to rule out any underlying conditions.

5. Recommendations for Improvement:
- Sleep Hygiene: Aim to establish a regular sleep schedule. Try to go to bed and wake up at the same time each day, ensuring you get adequate rest. This can help improve your overall health and may positively impact your sexual function.

- Stress Management: Engage in activities that help reduce stress, such as exercise, meditation, or hobbies that you enjoy. Finding healthy outlets for stress can improve your mental well-being and, in turn, your sexual health.

- Healthy Lifestyle: Maintain a balanced diet, stay hydrated, and engage in regular physical activity. These factors can significantly influence hormonal balance and overall health.

- Open Communication: If you continue to have concerns, consider discussing them with a trusted adult or healthcare provider. They can provide guidance and reassurance, and if necessary, conduct further evaluations.

In summary, while the changes you're experiencing can be concerning, they are often a normal part of adolescent development. By focusing on healthy habits and managing stress, you may find improvements in your sexual health. If symptoms persist or worsen, seeking medical advice would be a prudent step.
