Scar Issues After Nose Surgery: Itching and Inflammation Concerns - Dermatology

Scar issues

Hello doctor, I had nasal surgery three months ago, and the scar is located inside my nostrils and at the edge of my nostrils.
I would like to ask the doctor: 1.
It has been three months, and I have noticed that the scars feel itchier when the temperature is higher.
Why is that? Could higher temperatures (due to my workplace being quite warm) lead to scar hypertrophy or inflammation? 2.
Since the scars are located in my nostrils, could frequent forceful breathing cause tension on the scars, potentially leading to inflammation or hypertrophy? Thank you.

Li, 20~29 year old female. Ask Date: 2015/09/17

Dr. Wu Yuxin reply Dermatology

Based on the literal description, it is not possible to confirm the condition without actually seeing the lesion.
I recommend that you have the original surgeon take another look.

After undergoing nasal surgery three months ago, it is common to experience various sensations and concerns regarding the healing process, particularly related to scarring. Let's address your questions regarding itching and inflammation of the scars.

1. Itching of Scars in Warmer Temperatures: Itching is a common symptom during the healing process, especially in the early months following surgery. The sensation of itching can be exacerbated by increased temperatures, which can lead to increased blood flow to the area and heightened sensitivity of the skin. When the temperature rises, the body may react by increasing the metabolic activity in the skin, which can stimulate nerve endings and lead to itching sensations. Additionally, heat can cause the skin to become more hydrated, which may also contribute to the sensation of itchiness.

Regarding your concern about whether higher temperatures can lead to scar hypertrophy (thickening) or inflammation, it is important to note that while heat can exacerbate itching, it does not directly cause scarring to worsen. However, if the area becomes excessively warm and sweaty, it may increase the risk of irritation or infection, which could lead to inflammation. It is advisable to keep the area clean and dry, and if you notice any signs of infection (such as increased redness, swelling, or discharge), you should consult your surgeon or a healthcare provider.

2. Impact of Forceful Breathing on Scars: The location of your scars, particularly if they are at the nostrils, can indeed be affected by activities such as forceful breathing or excessive movement. When you breathe forcefully, the skin around the nostrils may stretch, which could potentially lead to tension on the healing scars. This tension can contribute to discomfort and may increase the risk of scar hypertrophy if the skin is repeatedly stressed during the healing process.

To minimize the risk of irritation or inflammation, it is advisable to avoid forceful breathing or any activities that may strain the area around the scars. Gentle breathing and avoiding excessive manipulation of the nose can help in the healing process. If you find that certain activities consistently lead to discomfort or itching, it may be beneficial to modify those activities or consult with your healthcare provider for personalized advice.

Additional Considerations:
- Scar Management: To promote optimal healing and minimize scar formation, consider using silicone gel sheets or topical silicone gel, which have been shown to improve the appearance of scars. Additionally, keeping the scars moisturized can help reduce itching and improve the overall texture of the skin.

- Follow-Up Care: Regular follow-up appointments with your surgeon are essential to monitor the healing process. They can provide tailored advice and interventions if any complications arise.

- Patience in Healing: Remember that scar maturation can take up to a year or more. During this time, it is normal for scars to change in appearance, texture, and sensitivity. If you have ongoing concerns about the scars, do not hesitate to reach out to your healthcare provider for reassurance and guidance.

In summary, while itching and inflammation can be concerning, they are often part of the normal healing process. By managing your environment, being mindful of activities that may stress the scars, and maintaining good scar care practices, you can support your healing journey effectively.
