Penile issues

Hello doctor, recently when I take a shower and retract the foreskin to clean the penis, I keep noticing a white layer that seems to be stuck between the penis and the foreskin.
My current approach is to pull the foreskin all the way back and gently wash away the white substance with water, but the issue reappears in about 1 to 2 days.
Sometimes, if I pull that white layer off too quickly, it can be painful.
Could you please tell me what this is? Is it something that has just started happening? By the way, I have not engaged in any sexual activity.

Li, 10~19 year old female. Ask Date: 2015/09/24

Dr. Xu Weikai reply Urology

Li: Hello, this is a normal phenomenon; it is a metabolic process of the epidermis.
It is essential to clean the skin daily.
When cleaning, use cool water only; avoid hot water, soap, or body wash.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
For now, try this approach.

The presence of a white discharge or a white, sticky substance under the foreskin is a common issue that many men experience, particularly those who are uncircumcised. This substance is often a combination of smegma, dead skin cells, and natural oils produced by the body. Smegma is a normal bodily secretion that can accumulate if proper hygiene is not maintained. It is important to understand that while smegma itself is not harmful, its accumulation can lead to irritation, inflammation, or infection if not cleaned regularly.

Causes of White Discharge Under the Foreskin
1. Smegma Accumulation: Smegma is produced by the sebaceous glands located in the skin of the penis. It is a mixture of skin cells, oils, and moisture. If the area is not cleaned regularly, smegma can build up and appear as a white, cheesy substance.

2. Poor Hygiene: Inadequate cleaning of the genital area can lead to the accumulation of smegma. It is essential to retract the foreskin and wash the area with mild soap and water regularly to prevent buildup.

3. Infections: Although you mentioned that you have not engaged in any sexual activity, infections can still occur. Conditions such as balanitis (inflammation of the glans) can cause discharge and discomfort. Symptoms may include redness, swelling, and pain.

4. Skin Conditions: Certain skin conditions, such as psoriasis or eczema, can also affect the genital area, leading to irritation and discharge.

Solutions and Recommendations
1. Maintain Proper Hygiene: It is crucial to practice good hygiene by gently retracting the foreskin and washing the area with warm water daily. Avoid using harsh soaps or cleansers that can irritate the sensitive skin.

2. Avoid Irritation: If you experience pain when removing smegma, be gentle. You can soak the area in warm water to soften the smegma before attempting to clean it. If the pain persists, it may be advisable to consult a healthcare provider.

3. Consult a Healthcare Provider: If the discharge continues to occur despite good hygiene practices, or if you notice any additional symptoms such as redness, swelling, or pain, it is important to seek medical advice. A healthcare provider can assess for any underlying infections or skin conditions that may require treatment.

4. Consider Circumcision: In some cases, men who experience recurrent issues with smegma and hygiene may consider circumcision. This surgical procedure removes the foreskin and can help prevent the accumulation of smegma and reduce the risk of infections.

5. Monitor Symptoms: Keep an eye on any changes in the discharge, such as color, consistency, or associated symptoms. If you notice any unusual changes, it is best to consult a healthcare professional.

Conclusion
In summary, the white discharge you are observing is likely smegma, which is a normal secretion. Maintaining proper hygiene is key to managing this issue. If you experience persistent symptoms or discomfort, it is advisable to consult a healthcare provider for further evaluation and management. Remember, taking care of your genital health is an important aspect of overall well-being.
