Which specialty should I see?

Hello, I would like to ask which specialty I should take my mother to see.
She is 57 years old and often tells me that she experiences unexplained tremors, has difficulty sleeping at night, and even when she does sleep, it feels like she is constantly waking up.
She has a history of gallstones and has been feeling weak, but despite eating, her digestive system seems unable to process food, leading to bloating.
We took her to a gastroenterologist who prescribed gastrointestinal medications and muscle relaxants, but they have not been effective.
She feels completely drained, and I really don't know what to do.
It's been two to three months, and I am at a loss.
Please tell me which specialty would be most helpful for her condition.

Ms. Zhang, 30~39 year old female. Ask Date: 2015/11/14

Hello, could you please have your mother's family physician evaluate her first, and then decide if a referral to another specialist is necessary? Thank you for your concern from Changhua Hospital.

It sounds like your mother is experiencing a complex array of health issues that are causing you and your family significant concern. Given her symptoms, including unexplained shaking, sleep disturbances, gastrointestinal problems, and a general feeling of weakness, it is crucial to approach her care from a multidisciplinary perspective.

1. Primary Care Physician (PCP): The first step should be to consult her primary care physician if she has one. A PCP can provide a comprehensive evaluation of her overall health and coordinate care among specialists. They can also perform necessary blood tests and other diagnostic procedures to rule out any underlying conditions.

2. Neurology: Since your mother is experiencing tremors and sleep disturbances, a neurologist should be consulted. Neurological issues can manifest in various ways, including tremors, and a neurologist can assess for conditions such as Parkinson's disease, essential tremors, or other movement disorders.

3. Gastroenterology: Given her history of gallstones and ongoing gastrointestinal symptoms, a gastroenterologist is essential. If the medications prescribed by her gastroenterologist have not been effective, it may be necessary to explore further diagnostic tests, such as an endoscopy or imaging studies, to determine if there are any structural issues or other gastrointestinal disorders contributing to her symptoms.

4. Psychiatry or Psychology: If your mother is experiencing significant anxiety or depression, which can often accompany chronic health issues, it may be beneficial to consult a psychiatrist or psychologist. Mental health professionals can provide support and treatment options that may help improve her overall well-being and coping strategies.

5. Endocrinology: If there are concerns about hormonal imbalances or metabolic issues, an endocrinologist may be necessary. Conditions such as thyroid disorders can lead to symptoms like fatigue, weight changes, and gastrointestinal problems.

6. Geriatrics: Since your mother is 57 years old, consulting a geriatrician could also be beneficial. Geriatricians specialize in the care of older adults and can help manage multiple health issues, ensuring that all aspects of her health are considered.

7. Nutritional Counseling: If she is experiencing digestive issues and has a poor appetite, consulting a registered dietitian may help. They can provide dietary recommendations that may alleviate some of her gastrointestinal symptoms and improve her overall nutrition.

8. Pain Management: If your mother is experiencing significant discomfort, a pain management specialist can help address her pain and improve her quality of life.

In summary, it is essential to take a comprehensive approach to your mother's health. Start with her primary care physician, who can guide you to the appropriate specialists based on her symptoms. It may also be helpful to keep a detailed record of her symptoms, medications, and any changes in her condition to share with her healthcare providers. This information can assist them in making a more accurate diagnosis and developing an effective treatment plan.
Additionally, consider involving family members in her care to provide emotional support and help her navigate the healthcare system. It can be overwhelming, but with the right support and medical attention, there is hope for improvement in her health and well-being.
