Endometrial polyp
Hello, doctor.
I experienced bleeding due to endometrial polyps and underwent a dilation and curettage (D&C) procedure.
I have been home for three days taking uterotonics and antibiotics.
The first two days had minimal bleeding, but on the third day, I noticed some brownish discharge, about the size of a fingertip, which is different from the previous red blood with clots.
I would like to ask: 1.
Is this normal postoperative bleeding? 2.
How long will it take for the bleeding to stop? 3.
Is it normal to experience occasional abdominal pain? 4.
Is there a possibility that the endometrium is starting to proliferate again? 5.
How long should I wait before engaging in intimate relations with my husband? Thank you, doctor.
Niming, 30~39 year old female. Ask Date: 2015/11/16
Dr. Lü Lizheng reply Obstetrics and Gynecology
1.
It may be bleeding from the uterine wound.
2.
Generally, the bleeding will stop within three days.
3.
It could be uterine contraction pain; if the pain worsens, it is necessary to return for an earlier check-up.
4.
The endometrium will regenerate, but it will not reach the level of hyperplasia.
5.
It is recommended to return for a follow-up in about a week, and only start sexual activity after the physician confirms that everything is normal.
Respectfully, Dr.
Li-Cheng Lu, Taoyuan Hospital.

Post-operative recovery after a uterine polyp removal can vary significantly among individuals, and it's essential to understand the typical healing process and what symptoms may be considered normal or concerning.
1. Is the bleeding normal?
After a uterine polyp removal, it is common to experience some bleeding as part of the healing process. The initial bleeding may be bright red, but as you described, the presence of brownish discharge (which is often old blood) is generally considered normal. This can occur as the body sheds any residual tissue and the uterus begins to heal. However, if the bleeding becomes heavy, bright red, or is accompanied by large clots, it would be advisable to contact your healthcare provider.
2. How long will the bleeding last?
The duration of post-operative bleeding can vary. Typically, light bleeding or spotting can last from a few days to a couple of weeks after the procedure. If you notice that the bleeding persists beyond two weeks or increases in volume, it is important to seek medical advice. Factors such as individual healing rates, the extent of the procedure, and overall health can influence recovery time.
3. Is abdominal pain normal?
Mild abdominal cramping or discomfort can be expected after the removal of a uterine polyp. This is due to the uterus contracting as it heals. However, if the pain is severe, persistent, or accompanied by other symptoms such as fever, chills, or foul-smelling discharge, it may indicate a complication, such as infection or improper healing, and should be evaluated by a physician.
4. Will the endometrium start to thicken again?
After a polyp removal, the endometrium (the lining of the uterus) can indeed begin to regenerate. This is a normal part of the menstrual cycle, and the endometrium will thicken in preparation for a potential pregnancy. However, if you have a history of endometrial hyperplasia or polyps, it may be beneficial to discuss with your doctor about monitoring your endometrial health through follow-up ultrasounds or biopsies as needed.
5. When can you resume intimate relations?
It is generally recommended to wait at least two weeks after a uterine polyp removal before resuming sexual intercourse. This allows time for the cervix to close and for any bleeding to subside. However, it is crucial to follow your doctor's specific recommendations based on your individual recovery process. If you experience any discomfort or unusual symptoms during or after intercourse, it is advisable to consult your healthcare provider.
In summary, while some bleeding and discomfort can be normal after a uterine polyp removal, it is essential to monitor your symptoms closely. If you have any concerns about the amount of bleeding, the nature of the pain, or any other symptoms, do not hesitate to reach out to your healthcare provider for guidance. They can provide personalized advice and reassurance based on your specific situation and medical history. Regular follow-ups are also important to ensure that your recovery is progressing as expected.
